研究目的：

探讨"微信平台＋家庭参与式"个案管理模式在脑卒中患者肢体功能康复中的应用效果  

Objectives of Study：

To explore the application effect of "Wechat platform + family participation" case management model in limb functional rehabilitation of stroke patients

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）头颅CT或MRI确诊的脑卒中患者； （2）遗留肢体功能障碍，肌力小于5级； （3）生命体征平稳； （4）患者或家属具备利用智能手机获取康复训练知识的能力

Inclusion criteria

(1) Stroke patients diagnosed by CT or MRI. (2) left limb dysfunction, muscle strength less than grade 5 (3) the vital signs are stable. (4) patients or their families have the ability to use smartphones to acquire rehabilitation training knowledge.

排除标准：

（1）卒中前有肢体功能障碍； （2）意识不清或精神障碍； （3）并存心、肺、肾等器官损伤； （4）无家属或家属认知异常（不能正常交流）、精神行为异常。

Exclusion criteria：

1) limb dysfunction before stroke (2) unconsciousness or mental disorder (3) complicated with heart, lung, kidney and other organ injuries. (4) No family members or family members had abnormal cognition (unable to communicate normally) and abnormal mental behavior.
