研究目的：

（1）编制医院-家庭过渡期老年慢性病患者参与用药安全行为问卷。 （2）了解医院-家庭过渡期老年慢性病患者参与用药安全行为现状。 （3）分析医院-家庭过渡期老年慢性病患者参与用药安全行为的影响因素。  

Objectives of Study：

(1) Compile a questionnaire on the participation of elderly patients with chronic diseases in the hospital-family transition period on medication safety behavior. (2) Understand the current situation of participation in medication safety behavior of elderly patients with chronic diseases in the hospital-family transition period. (3) The influencing factors of elderly patients with chronic diseases in the hospital-family transition period were analyzed.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.已被二级及以上医院确诊至少患为一种慢性病； 2.年龄≥60岁； 3.出院后至少使用1种药物治疗； 4.出院后返回至家庭后； 5.出院时间小于2个月； 6.知情同意后参与本研究。

Inclusion criteria

1. Has been diagnosed with at least one chronic disease by a second level or higher hospital; 2. Age ≥ 60 years old; 3. After discharge, use at least one medication for treatment; 4. After discharge and returning to home; 5. The discharge time is less than 2 months; 6. Participate in this study with informed consent.

排除标准：

1.出院后因病情需要转至其他医疗机构继续治疗者； 2.病情易发生变化的危急重症和（或）疾病终末期患者； 3.生活不能自理者； 4.听力、视力障碍或与他人沟通障碍者。

Exclusion criteria：

1. Those who need to be transferred to other medical institutions for further treatment due to their illness after discharge; 2. Critical and/or end-stage patients with easily changing conditions; 3. Those who cannot take care of themselves; 4. Individuals with hearing, visual impairment, or communication difficulties with others.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

由经过专门培训的资料收集员对数据进行测量和采集。采用Excel 2016建立数据库，为保证数据录入的准确性，所有数据均双人录入；采用SPSS软件进行数据处理。

Data are measured and collected by the professional training nurses; the database is established using excel 2016, and then to ensure the accuracy of data, all data is entered by two researchers; SPSS will be used for data analysis.
