研究目的：

孤独症谱系障碍（autism spectrum disorder, ASD）因其逐年攀升的患病率、高致残率、给社会及家庭带来沉重负担，是一项公共卫生问题。营养在内的环境因素在ASD发病机制中的研究是热点，也是尚未阐明的难点。申请人前期及他人研究表明ASD患者维生素A缺乏（vitamin A deficiency，VAD）率高，且与ASD严重程度密切相关；维生素A活性代谢产物视黄酸（retinoic acid，RA）干预ASD大鼠模型及ASD人群社交核心症状缓解。多发性硬化模型中RA可抑制Th17细胞表达及IL-17（IL-17A）的分泌，而Th17细胞及其分泌的IL-17（IL-17A）直接参与ASD发病。为此，申请人拟以前期工作为基础，通过人群横断面、前瞻队列研究及体外细胞实验，深入研究VAD是否通过加速Th17细胞表达、IL-17（IL-17A）的分泌，加重ASD症状，为维生素A用于ASD干预的新思路提供理论依据。  

Objectives of Study：

Autism spectrum disorder (ASD) is a public health problem with its increasing prevalence, high disability rate and heavy burden on society and families.Though many studies focused on the roles of environmental factors including nutrition in ASD ,it has not yet been well illustrated. The prevalence of vitamin A deficiency (VAD) in ASD patients was high, and it was significantly related to the severity of ASD; Retinoic acid (RA), an active metabolite of vitamin A, could improve autism symptoms in both autistic human and rat. In multiple sclerosis model, RA can inhibit the expression of Th17 cells and the secretion of IL-17 (IL-17A), while Th17 cells and their secreted IL-17 (IL-17A) are directly involved in the pathogenesis of ASD. We aimed to explore whether VAD can aggravate ASD symptoms by accelerating the expression of Th17 cells and the secretion of IL-17 (IL-17A),with cross-section, prospective cohort study and in vitro cell experiment, which will provide evidence for future studies of neuropathological mechanism and intervention of vitamin A in ASD.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

ASD组纳入标准：1）经过我院发育行为专科医师确诊，符合美国精神疾病诊断与统计手册第5版（DSM-5）对ASD的诊断标准；2）结合孤独症诊断观察量表（autism diagnostic observation schedule, ADOS）阳性；3）18月-6岁；4）初诊及复诊均可。
对照组入组标准：1）不符合美国精神疾病诊断与统计手册第5版（DSM-5）诊断标准；2）18月-5岁；3）典型发育正常儿童；4）智力筛查（ASQ或PPVT）或诊断性发育商（儿心）测评未提示发育偏离；5）孤独症筛查阴性（ABC量表）。

Inclusion criteria

Inclusion criteria for ASD: 1) Diagnosed by development behavior pediatrician, meeting the diagnostic criteria of DSM-5 for autism spectrum disorder; 2) with autism diagnostic observation schedule (ADOS) positive; 3) 18-6 years old; 4) Both primary and secondary diagnosis are acceptable.
Inclusion criteria for CON: 1) did not meet the diagnostic criteria of DSM-5 for all the mental diorders; 2) 18-6 years old; 3) Typical developing children; 4) Intelligence screening (ASQ or PPVT) or diagnostic developmental quotient (ECQ) assessment did not indicate developmental deviation; 5) Autism screening was negative (ABC scale).

排除标准：

ASD排除标准 共患1）其他神经发育障碍注意缺陷多动障碍抽动障碍语音障碍、特定学习障碍；2）其他精神障碍：焦虑障碍、抑郁障碍、双相情感障碍、行为障碍、强迫及相关障碍、创伤及应激相关障碍、分离障碍、躯体症状及相关障碍、睡眠-觉醒障碍、破坏性、冲动型控制及品行障碍等精神障碍；3）器质性疾病，如癫痫、矮小、先天性心脏病、遗尿症、免疫性脑炎、免疫缺陷等；4）其他有孤独症表现的综合征：Rett 综合征、脆性X综合征；5）近1月使用营养补充剂（维生素D除外），近2周有药物使用史。
对照组排除标准：同ASD排除标准1）到5）条。

Exclusion criteria：

Exclusion criteria for ASD: 1) other neurodevelopmental disorders, ADHD, tic disorder, speech disorder, specific learning disorder; 2) Other mental disorders: anxiety disorder, depression disorder, bipolar disorder, behavioral disorder, compulsive and related disorder, trauma and stress-related disorder, separation disorder, physical symptoms and related disorder, sleep-wake disorder, destructive, impulsive control and conduct disorder, and other mental disorders; 3) Organic diseases, such as epilepsy, short stature, congenital heart disease, enuresis, immune encephalitis, immune deficiency, etc; 4) Other syndromes with autism: Rett syndrome, fragile X syndrome; 5) Use nutritional supplements (except vitamin D) in the past 1 month, and have a history of drug use in the past 2 weeks.
Exclusion criteria for CON: the same as those of ASD 1) to 5).

MVAD组请第三方进行随机数字表的设计[使用EXCEL表里的RANDBETWEEN函数，随机数字1-6代表不同组合的区组，每个区组4例被试，字母A代表VA干预，B代表安慰剂干预]进行VA或安慰剂干预。
