研究目的：

①分析总结鼻咽癌患者外周插入PICC管合并静脉血栓栓塞的发生率，为临床使用PICC管提供参考依据。 ②使用生信信息学方法进一步探讨鼻咽癌患者外周插入PICC管合并静脉血栓栓塞发生的潜在作用机制，为后续基础研究提供理论基础。 ③建立PICC置管后静脉血栓发生预测模型。  

Objectives of Study：

① Analysis and summarize the incidence of peripheral PICC tube insertion with venous thromboembolism, to provide a reference for clinical use of PICC tubes. ② The student information method was used to further explore the potential mechanism of action of peripheral insertion of PICC tubes with venous thromboembolism in NPC patients, to provide a theoretical basis for subsequent basic research. ③ Establish a prediction model of venous thrombosis after PICC catheterization.

药物成份或治疗方案详述：

①鼻咽癌患者外周插入PICC管合并静脉血栓栓塞的发生率 1.回顾性收集来自本中心2019-2023年鼻咽癌临床收治病例，全部病例均通过临床、影像学、病理诊断，随访资料齐全。 2.通过影像学方法（静脉彩色多普勒超声）检查病例的静脉血栓血栓栓塞，分别设立阴性对照、肿瘤对照、静脉栓塞对照。 3.SPSS16.0统计软件对静脉血栓栓塞与临床病例进行关联性分析和预后统计分析，了解静脉栓塞发生情况与相关临床资料之间的关系，单因素生存分析对临床各项指标与鼻咽癌生存期进行相关性分析，建立Cox比例风险回归模型，综合探讨临床各因素与鼻咽癌外周插入PICC管静脉血栓栓塞发生及预后的相关性。 ②鼻咽癌患者外周插入PICC管合并静脉血栓栓塞的发生分子机制 1.通过GEO、GTX以及TCGA等数据库基因芯片Microarray分析鼻咽癌患者发生血栓静脉栓塞后基因表达谱变化情况，对原始数据进行归一化、标准化，差异表达分析，明确无血栓静脉栓塞后表达发生改变的下游调控分子，找出芯片结果中有显著差异的潜在作用分子。 2.使用GO和STRING分析显著差异的潜在作用分子的功能富集集群，找出与血栓静脉栓塞相关的潜在作用分子，并构建蛋白互作网络。 3.使用K-M曲线分析潜在作用分子与鼻咽癌外周插入PICC管合并静脉血栓栓塞患者的预后相关性，使用单变量Cox回归找出预后分子，并采用Logistic回归模型和套索变量选择和收缩方法,利用R语言程序软件包glmnet4.0缩小mRNA表达谱。在运行n倍值等于10的主算法之前，通过交叉验证例程确定惩罚正则化参数k。通过使用λmin(λ的值)来最终确定k值，给出最小的平均交叉验证误差。将数据随机分为训练集和测试集。最后，建立了多因素Cox回归模型。此外，基于训练组的模型在试验组通过ROC和诺模图进行了验证。 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）鼻咽癌，无论其病因，病理学支持诊断；
（2）年龄大于 18 岁；
（3）签署知情同意书。

Inclusion criteria

(1) nasopharyngeal carcinoma, regardless of its etiology, pathology supports the diagnosis; (2) Age is over 18 years old; (3) Sign the informed consent form.

排除标准：

（1）临床资料不全的患者。
（2）肢体骨折史；
（3）有抗凝药物禁忌症者，严重肝脏、肾脏疾病者；
（4）入院前有静脉血栓史的患者；
（5）严重肝脏、肾脏疾病者；
（6）合并其他肿瘤患者。

Exclusion criteria：

(1) Patients with incomplete clinical data. (2) History of limb fracture; (3) Patients with contraindications to anticoagulant drugs, and patients with serious liver and kidney diseases; (4) Patients with a history of venous thrombosis before admission; (5) Serious liver and kidney diseases; (6) Patients with other tumors.
