研究目的：

本研究将通过临床实验研究方法，验证连续性高频振荡治疗对机械通气患者肺通气的影响，为其进一步推广应用提供科学可靠的理论研究基础。  

Objectives of Study：

This study will verify the effect of Continuous High-Frequency Oscillation Therapy on lung aeration in mechanically ventilated patients through clinical experimental research methods, and provide a scientific and reliable theoretical research basis for its further application.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.18-80岁 2.GCS≤8分(言语反应为1分) 3.有创机械通气≥48h 4.胸部CT、胸部X片或肺部超声证实存在肺不张

Inclusion criteria

1. 18-80 years old 2. GCS ≤ 8 points (verbal response is 1 point) 3. Invasive mechanical ventilation for ≥48h 4. Chest CT, chest X-ray or lung ultrasound confirming the presence of pulmonary atelectasis

排除标准：

1.预计人工气道保留<5天 2.BMI≥40kg/m2 3.ICP＞22mmHg 4.严重脊柱、胸廓畸形 5.严重呼吸系统基础疾病需要长期家庭氧疗或无创通气 6.已知或疑似膈肌失能如：膈神经断裂、神经肌肉疾病等 7.未处理的张力性气胸 8.肺内活动性出血 9.妊娠 10.严重血流动力学不稳（2h内增加血管活性药物剂量或MAP≤65mmHg） 11.严重的其他器官功能障碍预计短期内死亡（7天）或姑息性治疗 12.医疗团队认为其他不适合开展治疗的原因 13.未签署知情同意书

Exclusion criteria：

1. expected artificial airway maintenance <5 days 2. BMI ≥40kg/m2 3. ICP>22mmHg 4. severe spinal or thoracic deformity 5. Severe underlying respiratory disease requiring long-term home oxygen therapy or non-invasive ventilation 6. Known or suspected diaphragmatic dysfunction e.g. phrenic nerve rupture, neuromuscular disease 7. Untreated tension pneumothorax 8. Active intrapulmonary haemorrhage 9. Pregnancy 10. severe haemodynamic instability (increased dose of vasoactive drugs within 2h or MAP ≤ 65mmHg) 11. severe other organ dysfunction with expected short-term death (7 days) or palliative care 12. Other reasons deemed by the medical team to be inappropriate for treatment 13. Failure to sign an informed consent form
