研究目的:

近年越来越多的研究显示,lncRNA在阿尔兹海默症等老年神经系统疾病患者中存在异常表达并参与了多种生物学过程和通路,提示lncRNA可能在神经系统疾病的发生发展中有重要的调控作用,并提出了lncRNA作为潜在的神经系统疾病治疗靶点和生物标志物的可能性。因此,探讨更多的lncRNA作为认知功能受损诊断生物学标志的潜在价值以及lncRNA联合诊断认知功能受损的临床应用价值,可丰富临床中对认知功能受损的认识,为认知功能受损早期诊断提供参考和探索更好的诊断工具及新的治疗靶点。 越来越多的证据表明,炎症反应在阿尔兹海默症和轻度认知功能受损等神经退行性病变中起着关键作用。探讨炎症标志物与老年人认知功能受损的关系;分析外泌体lncRNA的异常表达、炎症标志物和老年人发生认知功能受损之间的中介作用,为老年人认知功能受损早预防、早发现和早治疗提供参考依据。  

Objectives of Study:

In recent years, more and more studies have shown that lncRNAs are abnormally expressed and involved in a variety of biological processes and pathways in patients with age-related neurological disorders such as Alzheimer's disease, suggesting that lncRNAs may have important regulatory roles in the development of neurological disorders and raising the possibility that lncRNAs may be used as potential therapeutic targets and biomarkers for neurological disorders. Therefore, exploring the potential value of more lncRNAs as diagnostic biomarkers of cognitive impairment as well as the clinical application value of lncRNA co-diagnosis of cognitive impairment may enrich the understanding of cognitive impairment in the clinic, provide references for early diagnosis of cognitive impairment and explore better diagnostic tools and new therapeutic targets. Increasing evidence suggests that inflammatory response plays a key role in the neurodegenerative pathology of Alzheimer's disease and mild cognitive impairment. To explore the relationship between inflammatory markers and cognitive impairment in the elderly; to analyze the mediating role between abnormal expression of exosomal lncRNA, inflammatory markers and the occurrence of cognitive impairment in the elderly, and to provide a reference basis for the early prevention, detection and treatment of cognitive impairment in the elderly.

纳入标准:

纳入标准:1)研究对象均大于等于65岁;2)病例组患者需符合认知功能受损标准:文盲者、小学者和中学及以上者的MMSE评分分别≤17分、≤20分和≤24分;3)对照组需符合认知功能正常标准:文盲者、小学者和中学及以上者的MMSE评分分别大于17分、20分和24分;4)本人及家属均知情同意,并自愿签署同意书。

Inclusion criteria

Inclusion Criteria: 1) The study subjects were all older than or equal to 65 years old; 2) The patients in the case group had to meet the criteria of impaired cognitive function: the MMSE scores of the illiterate, the elementary school students, and the secondary school and above were ≤17, ≤20, and ≤24, respectively; 3) The patients in the control group had to meet the criterion of normal cognitive function: the MMSE scores of the illiterate, the elementary school students, and the secondary school and above were greater than 17, 20, and 24, respectively. (4) The study subjects themselves and their families gave informed consent and voluntarily signed the consent form.

排除标准:

排除标准:1)存在其他中枢神经系统疾患,如脑肿瘤、脑创伤、中枢神经系统炎性或脱髓鞘性疾患,以及正常颅压脑积水等;2)存在其他引起认知功能下降的系统性疾病;3)存在显著的精神疾病(如精神分裂症和严重的抑郁症等),以及药物使用障碍或酒精滥用;4)存在脑血管意外等其他重大疾病;5)有严重的听力、视力障碍而不能完成问卷调查。

Exclusion criteria:

Exclusion criteria: 1) Presence of other central nervous system disorders, such as brain tumors, traumatic brain injury, inflammatory or demyelinating disorders of the central nervous system, as well as normal cranial pressure hydrocephalus; 2) Presence of other systemic diseases causing cognitive decline; 3) Presence of significant psychiatric disorders (e.g., schizophrenia and major depression, etc.), as well as substance use disorders or alcohol abuse; 4) Presence of other major diseases, such as cerebral vascular accident; 5) Presence of severe mental disorders (e.g., schizophrenia and severe depression, etc.) other major illnesses; 5) the presence of severe hearing or vision impairment that prevents completion of the questionnaire.
