研究目的：

1. 评估胸腔镜精准肺切除术联合实时影像引导下经皮肺消融术（简称为肺部杂交手术）治疗老年肺部多发结节的可行性，优化围术期管理要点，建立标准化手术流程。 2. 随访手术中安全性事件、术后并发症和死亡率，评估肺部杂交手术应用于老年患者的安全性。 3. 动态评估老年患者术后呼吸功能和手术相关生活质量，随访肿瘤局部复发率和复发模式，验证肺部杂交手术在老年患者中的应用价值。 4. 建立老年肺多发结节患者数据库，综合衡量老年脏器功能、手术风险与获益，分析影响手术安全性和预后的因素，优化手术策略，探索适用于老年肺多发结节患者的个性化诊疗模式。  

Objectives of Study：

1. To evaluate the feasibility of thoracoscopic pneumonectomy combined with real-time image-guided percutaneous lung ablation (referred to as pulmonary hybrid surgery) for the treatment of multiple pulmonary nodules in the elderly patients, optimize the key points of perioperative management, and establish a standardized surgical procedure. 2. to follow up intraoperative safety events, postoperative complications and mortality, and to assess the safety of pulmonary hybridization surgery applied to elderly patients. 3. To dynamically assess the postoperative respiratory function and surgery-related quality of life in elderly patients, follow up the local tumor recurrence rate and recurrence pattern, and verify the value of pulmonary hybridization surgery applied in elderly patients. 4. To establish a database of elderly patients with multiple pulmonary nodules, to comprehensively measure elderly organ function, surgical risks and benefits, to analyze factors affecting surgical safety and prognosis, to optimize surgical strategies, and to explore personalized treatment models applicable to elderly patients with multiple pulmonary nodules.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、年龄≥65周岁；
2、高分辨率胸部螺旋CT显示同侧肺不同肺叶或双侧不同肺叶的多发肺结节，经胸外科医师评估肺结节临床和影像特征，参照诊断多原发肺癌的Martini-Melamed标准，临床诊断为同时性多原发性肺癌，而非转移性肺癌；
3、按卫生部《原发性肺癌诊疗规范（2018年版）》完成肿瘤分期检查，经过胸外科医师评估肺部肿瘤为可切除，每个肺结节单独进分期，临床分期为I期-IIIa期，符合手术适应症；
4、术前全身脏器功能评估显示患者可耐受胸腔镜肺切除术和经皮肺消融术；
5、患者自愿入组本研究，并签署知情同意书；

Inclusion criteria

1, Age ≥ 65 years.
2, high-resolution chest CT showing multiple pulmonary nodules in different lobes of the ipsilateral lung or in different lobes of the bilateral lung, with clinical diagnosis of simultaneous multiple primary lung cancer after assessment of the clinical and imaging features of the lung nodules by a thoracic surgeon, with reference to the Martini-Melamed criteria for the diagnosis of multiple primary lung cancer.
3. the lung nodules were resectable after tumor staging examination and evaluation by a thoracic surgeon, each lung nodule was individually staged, and the clinical stage was stage I-IIIa, eligible for surgery.
4. preoperative systemic organ function assessment indicated that the patient could tolerate thoracoscopic pulmonary resection and percutaneous pulmonary ablation.
5. the patient voluntarily participated in this study and signed an informed consent form.

排除标准：

1、肺部病灶诊断为良性；
2、肺部病灶诊断为小细胞肺癌；
3、合并身体其他部位的恶性肿瘤，或既往有其他恶性肿瘤病史；
4、经胸外科医师评估，肺结节诊断为转移性肺癌，而非多原发性肺癌；
5、按卫生部《原发性肺癌诊疗规范（2018年版）》完成肿瘤分期检查，经过胸外科医师评估肺部病灶为不可切除，临床分期达到IIIb期-IV期；
6、经过胸外科医师评估，因合并胸膜腔粘连、胸壁畸形等因素，肺部病灶无法行胸腔镜肺切除术，需直接行开胸手术；
7、患者既往曾接受过肺切除术、肺部消融术、胸壁手术、胸骨或肋骨手术等胸部手术；
8、患者既往曾接受过胸部放疗、化疗、靶向等治疗；
9、术前全身脏器功能评估显示患者存在麻醉或手术禁忌症，无法耐受肺切除术或经皮肺消融术；

Exclusion criteria：

1. Pulmonary lesions were diagnosed as benign.
2 Pulmonary lesions were diagnosed as small cell lung cancer.
3, The co-existence of malignancy of other organs, or a previous history of other malignancies
4. pulmonary nodules were diagnosed as metastatic lung cancer rather than multiple primary lung cancers after the assessment by a thoracic surgeon
5 After completion of tumor staging examination, the pulmonary lesions were assessed by the thoracic surgeon as unresectable with clinical stage IIIb-IV.
6、 Due to the combination of pleural cavity adhesions, chest wall deformities and other factors, the pulmonary lesion is not amenable to thoracoscopic pneumonectomy and requires direct open thoracotomy.
7, The patient has undergone previous thoracic surgery such as pneumonectomy, lung ablation, chest wall surgery, sternal or rib surgery
8, the patient has received previous treatment such as chest radiotherapy, chemotherapy, targeted therapy, etc.
9. the preoperative systemic organ function assessment shows that the patient has contraindications to anesthesia or surgery and is unable to tolerate pneumonectomy or percutaneous lung ablation
