研究目的：

1. 在本项目组具有自主知识产权的发明专利“一种粪菌定位移植方法”（专利号：ZL201810866487.X）基础上，进一步优化和完善肠道菌群分离提取关键技术和全流程技术参数设置，实现移植物定性定量。 从数量上、全菌群种类上保证FMT植入的一致性，为FMT疗效的评价提供了可靠的前提基础。 2. 在本项目组2017年通过总医院医疗新技术临床应用审批的“肠道菌群胶囊移植治疗重度食物不耐受相关疾病的临床应用”（伦审第S2017-002-03号）项目基础上，以肠道菌群胶囊定位移植关键技术为手段，重点开展针对肠易激综合征（Irritable Bowel Syndrome, IBS）患者的粪菌移植临床干预工作，进一步明确粪菌移植对肠易激综合征的治疗作用。  

Objectives of Study：

1. Further optimize and improve the key technology of intestinal microbiota isolation and extraction to achieve the qualitative and quantitative graft. The consistency of FMT implantation was ensured in terms of number and whole flora species, which provided a reliable premise and basis for the evaluation of FMT efficacy. 2. On the basis of qualitative and quantitative implant, clinical observational studies on the treatment of irritable bowel syndrome (diarrhea type and constipation type) with fecal bacteria transplantation were conducted, in order to find the phenotypic characteristics of patients with effective FMT treatment.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

在项目组前期工作基础上，拟招募入组IBS人群70例，其中，IBS便秘型（IBS-C）和IBS腹泻型（IBS-D）各35例。1、纳入标准（1）国籍：中国；（2）年龄：18岁-60岁；（3）符合IBS各亚型的罗马IV诊断标准。

Inclusion criteria

Based on the preliminary work of the project team, 70 cases of IBS were planned to be recruited, among which 35 cases were of IBS constipation type (IBS-C) and 35 cases of IBS diarrhea type (IBS-D). (1) Nationality: Chinese; (2) Age: 18-60 years old; (3) The Rome IV diagnostic criteria for each subtype of IBS were met.

排除标准：

（1）近3个月内抗生素和/或免疫抑制剂和/或益生菌和/或泻药（含减肥药）用药史； （2）先天性或获得性免疫缺陷病患者或严重免疫抑制者（中性粒细胞<1500个/mm3，淋巴细胞<500个/mm3）； （3）肠黏膜严重破坏者或未能明确肠黏膜损伤病因者； （4）当前诊断为暴发性结肠炎或中毒性巨结肠者； （5）因存在严重腹泻、显著纤维性肠腔狭窄、严重消化道出血、高流量肠瘘等原因，无法耐受50%热卡需求的肠内营养者； （6）合并肠外器官感染，必须使用广谱抗生素干预者； （7）合并严重营养不良（体质指数<15kg/m2），严重低蛋白血症者（白蛋白<25g/L）； （8）重度过敏体质者； （9）处于备孕期或怀孕期或哺乳期女性； （10）生活无法自理或存在严重心理或行动障碍，不能主动配合完成干预者。

Exclusion criteria：

(1) Use of antibiotics and/or immunosuppressants and/or probiotics and/or laxatives (including diet drugs) within the last 3 months; (2) Congenital or acquired immune deficiency patients or severely immunosuppressed patients (neutrophils <1500 /mm3, lymphocytes <500 /mm3); (3) Severe damage of intestinal mucosa or failure to identify the cause of intestinal mucosal injury; (4) Patients currently diagnosed with fulminant colitis or toxic megacolon; (5) enteral nutrition patients who cannot tolerate 50% of heat calorie requirements due to severe diarrhea, significant fibrous intestinal stricture, severe gastrointestinal bleeding, high-flow intestinal fistula and other reasons; (6) Patients with parenteral organ infection requiring broad-spectrum antibiotic intervention; (7) Patients with severe malnutrition (body mass index <15kg/m2) and severe hypoproteinemia (albumin <25g/L); (8) severe allergic constitution; (9) Women who are preparing for pregnancy or during pregnancy or breastfeeding; (10) Those who are unable to take care of themselves or have serious psychological or action disorders and cannot actively cooperate to complete the intervention.
