探究脑深部电刺激(DBS)对帕金森病(PD)伴快动眼睡眠障碍(RBD)患者的睡眠期电生理的影响;探究不同靶点刺激对PD伴RBD患者的睡眠期电生理的调节差异;明确不同参数刺激对PD伴RBD患者的睡眠期电生理的调节差异。  

Objectives of Study:

Exploring the effects of deep brain electrical stimulation (DBS) on the sleep electrophysiology of Parkinson's disease (PD) patients with rapid eye movement sleep disorder (RBD); Exploring the differences in the regulation of sleep electrophysiology in PD patients with RBD by different target stimuli; Clarify the differences in the regulation of sleep electrophysiology in PD patients with RBD by different parameter stimuli.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

① 于天坛医院功能神经外科住院行DBS手术治疗的根据英国脑库标准诊断的PD患者;② 术前根据快动眼睡眠障碍筛查量表诊断为临床RBD(得分≥5);③ 年龄35-70岁;④ 行双侧STN-DBS手术或双侧GPi-DBS手术;⑤ 围手术期没有严重不良反应如颅内出血、颅内感染。

Inclusion criteria

① PD patients diagnosed according to British Brain Bank standards who were hospitalized in the functional neurosurgery department of Tiantan Hospital for DBS surgery; ② Preoperative diagnosis of clinical RBD based on the Rapid Eye Movement Sleep Disorder Screening Scale (score ≥ 5); ③ Age 35-70 years old; ④ Perform bilateral STN-DBS surgery or bilateral GPi-DBS surgery; ⑤ There were no serious adverse reactions such as intracranial hemorrhage or infection during the perioperative period.

排除标准:

① 存在严重精神、认知、心理障碍,不能签署知情同意书或无法配合手术及各项任务;② 存在神经外科手术禁忌症,如脑积水、脑萎缩、脑血管病后遗症、心脏病等心脑血管疾病;③ 存在肿瘤、肝功肾功严重异常(指标超过正常值三倍)等严重影响健康的伴随性疾病;④ 幽闭恐惧、体内有植入物,影响核磁扫描;⑤ 术前评估中MMSE<22或MoCA<15,提示存在严重痴呆的PD患者。

Exclusion criteria:

① Having serious mental, cognitive, and psychological disorders, unable to sign informed consent forms, or unable to cooperate with surgeries and various tasks; ② There are contraindications to neurosurgery, such as hydrocephalus, brain atrophy, sequelae of cerebrovascular disease, heart disease and other cardiovascular and cerebrovascular diseases; ③ There are accompanying diseases that seriously affect health, such as tumors and severe abnormalities in liver and kidney function (indicators exceeding three times the normal value); ④ Fear of claustrophobia, presence of implants in the body, affecting magnetic resonance imaging; ⑤ The preoperative evaluation showed that MMSE<22 or MoCA<15, indicating the presence of severe dementia in PD patients.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

研究者为每个受试者建立独立的研究编号,用研究编号命名相应数据资料,并删除所有数据中的患者姓名、住院号信息。研究资料包括量表评分、电生理数据,dicom格式的磁共振数据等。 所有数据经加密,由项目负责人保管。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Researchers establish an independent study number for each subject, name the corresponding data with the study number, and delete all patient names and hospitalization numbers from the data. Research materials include scale scores, electrophysiological data, and magnetic resonance imaging data in DICOM format. All data is encrypted and kept by the project leader.
