研究目的：

本研究的首要目标是评估指南转化流程是否能改善基层血脂异常患者的LDL-C控制水平。指南转化流程包括对基层医护人员加强培训教育等多重手段进行血脂异常防治。该干预措施强调通过综合性医院和社区卫生服务中心的上下技术沟通，医疗和公共卫生的医防结合，在社区卫生服务中心通过培训、质控和反馈，开展人群血脂异常综合管理。  

Objectives of Study：

The primary objective of this study was to evaluate whether the guideline transformation process can improve the control level of LDL-C in patients with dyslipidemia. The transformation process of the guideline includes multiple means, such as strengthening the training and education of grassroots medical staff. The intervention measures emphasize technical communication between general hospitals and community health service centers, integration of medical treatment and public health, providing training, quality control, and feedback at community health service centers, and thus implement comprehensive management of dyslipidemia in the targeted population.

药物成份或治疗方案详述：

干预措施分为2步进行，首先对中国成人血脂异常防治指南中的基层适宜技术进行筛选，通过流程化进行技术规范制定，把血脂异常基层管理技术细分为：早期筛查技术、诊断技术、危险分层评估技术、分级管理技术、随访管理技术、药物治疗技术和自我管理技术[9]。同时配套规范制定医务人员培训教育课件和患者健康教育课件。 其次，对参与项目的基层医务人员开展线上线下相结合培训，包括血脂异常防治知识培训、技术规范培训和项目实施的管理培训。每三个月开展项目业务培训和患者健康教育能力培训。 对照组仅开展项目实施管理培训，血脂异常管理基于基层医务人员指南学习开展防控工作。 两组分别在基线、3、6、9、12、15、18、21和24个月进行随访，在基线和24个月开展心血管病风险综合评估。 

Description for medicine or protocol of treatment in detail：

intervention The intervention measures are divided into two steps. First, screening the for grassroots appropriate technologies, and developing the process-oriented technical specification. The primary management technologies of dyslipidemia are subdivided into early screening, diagnostic, risk stratification, hierarchical management, follow-up management, drug treatment, and self-management technology [9]. At the same time, medical staff training, and patient education courseware will be formulated with norms. Secondly, a combination of online and offline training will be carried out for primary medical staff participating in the project, including the prevention and treatment of dyslipidemia, technical specifications, and management training on project implementation. Project implementation training and patient health education ability training will also be provided every three months. The control group will only be provided with project implementation management training. The management of dyslipidemia will be based on the primary medical staffs self-learning of the guideline. Both groups will be followed up at baseline, 3, 6, 9, 12, 15, 18, 21, and 24 months respectively, and a comprehensive assessment of cardiovascular disease risk will be carried out at baseline and 24 months. 

纳入标准：

1、年龄：35-75岁，
2、符合血脂异常诊断标准： （1）在未服用调脂药物情况下，血清检测指标符合下列一项或以上者：①总胆固醇（TC）≥6.2mmol/L；②低密度脂蛋白胆固醇(LDL-C)≥3.4mmol/L；③甘油三酯（TG）≥2.3mmol/L。（2）既往有血脂异常史，目前正在服用调脂药物的人员。
3、能配合项目随访管理并签署知情同意书。

Inclusion criteria

1. Age: 35-75 years old
2. Meet the diagnostic criteria for dyslipidemia: (1), Serum test meet one or more of the followings without taking lipid-lowering drugs: ① total cholesterol (TC) ≥ 6.2mmol/L; ② low-density lipoprotein cholesterol (LDL- C)≥3.4mmol/L; ③Triglyceride (TG)≥2.3mmol/L. (2) People who have a history of dyslipidemia in the past and are currently taking lipid-lowering drugs.
3. Able to cooperate with project follow-up plan and willing to sign informed consent form.

排除标准：

1、近半年患有急性心肌梗塞、脑卒中患者，
2、慢性肾功能不全4期及以上，
3、严重肝功能不全者 （ALT或AST升高超过正常上限3倍），
4、预期生存时间小于1年者。
5. 正在参与其他干预性的临床试验。

Exclusion criteria：

1. Patients with acute myocardial infarction or stroke in the past six months,
2. Chronic renal insufficiency, stage 4 and above,
3. Patients with severe hepatic insufficiency (ALT or AST increased more than 3 times the upper limits of normal),
4. The life expectancy is less than 1 year.
5. Participating in other interventional clinical trials.
