本研究的目的,是探究院内3周动态平衡训练对卒中后偏瘫患者步行功能的影响效果及潜在机制。  

Objectives of Study:

The purpose of this study was to explore the effect and potential mechanism of 3 weeks of in-hospital dynamic balance training on walking function in patients with poststroke hemiplegia.

药物成份或治疗方案详述:

将受试者随机分为实验组(动态平衡训练+常规康复训练)和对照组(常规康复训练)。对照组进行常规康复训练,由运动疗法、物理因子治疗、作业疗法、手工能训练、针灸等项目组成,各项治疗均由专业治疗师根据患者具体情况给予个性化定制,但尽可能保持一致。运动疗法中包括肌力训练、关节活动度训练、平衡功能训练等、步态训练等,物理因子治疗包括肌电电生物反馈、神经肌肉电刺激、全身垂直律动治疗等。作业治疗包括任务导向性日常生活活动训练、有氧训练等,手工能训练包括手指活动度训练,手指协调性训练等。 在对照组,研究期间除常规的作业治疗、运动治疗以及理疗外,不进行额外的训练,只需在入组时以及干预结束后进行各项评估,需要持续佩戴加速度计记录体力活动水平。在实验组,运动干预频度和时间是在原有治疗方案基础上,另加动态平衡训练(共计3周,每周7次,每次30分钟),根据患者的反应进行合适强度的训练,由易到难进行训练。站位平衡二级以上的受试者采用站位动态平衡训练,采用意大利Tecnobody平衡测试及训练系统(型号:PK252)。动态平衡干预内容:采用系统内置的单脚控制重心点前后位移、单脚控制重心点左右位移以及斜向位移,以及单脚控制重心完成范围内画圈的任务等。后期通过调整阻力、延长训练时间及缩短组间间隙的方式增加训练难度与强度。每个任务设定在2分钟,总时长控制在30分钟左右。在3周训练前后进行各项评估。 

Description for medicine or protocol of treatment in detail:

Subjects were randomly divided into experimental group (dynamic balance training + routine rehabilitation training) and control group (routine rehabilitation training). The control group received routine rehabilitation training, which consisted of exercise therapy, physical factor therapy, occupational therapy, manual energy training, acupuncture and moxibustion, etc. All treatments were personalized and customized by professional therapists according to the specific conditions of patients, but as far as possible, they were consistent. Exercise therapy includes muscle strength training, range of motion training, balance function training, gait training, etc., while physical factor therapy includes electromyographic biofeedback, neuromuscular electrical stimulation, and systemic vertical rhythm therapy. Occupational therapy includes task-oriented activities of daily living training, aerobic training, etc. Manual training includes finger range of motion training, finger coordination training, etc. In the control group, no additional training was provided during the study except for routine occupational therapy, exercise therapy, and physical therapy. All assessments were made at the time of enrollment and after the intervention, and physical activity levels were continuously recorded with an accelerometer. In the experimental group, the frequency and time of exercise intervention were based on the original treatment scheme, plus dynamic balance training (a total of 3 weeks, 7 times a week, 30 minutes each time), and appropriate intensity training was conducted according to patients' responses, from easy to difficult. Subjects with standing balance level 2 or above were trained in standing dynamic balance, and the Italian Tecnobody balance test and training system (Model: PK252) was adopted. Content of dynamic balance intervention: the built-in single-foot control of the forward and backward displacement of the center of gravity, the left-right displacement of the center of gravity and the oblique displacement of the center of gravity, and the single-foot control of the center of gravity to complete the task of drawing circles within the range, etc. In the later stage, the training difficulty and intensity were increased by adjusting the resistance, extending the training time and shortening the interval between groups. Each task is set to be 2 minutes, and the total time is controlled to be about 30 minutes. Each assessment was performed before and after 3 weeks of training. 

纳入标准:

①年龄≥45岁;
②符合1995年全国第四届脑血管疾病学术会议修订的诊断标准,经颅脑CT、MRI检查确诊的患者;
③单侧,首次发病患者或既往有发作但未遗留神经功能障碍;
④意识清楚,生命体征稳定,病情稳定;
⑤能够理解治疗师或仪器指令,并能完成所要求的的动作;
⑥简易三级平衡评定法站坐位二级及以上。

Inclusion criteria

① Age ≥45 years old;
② Patients who met the diagnostic criteria revised at the Fourth National Conference on Cerebrovascular Diseases in 1995 and were diagnosed by craniocerebral CT and MRI;
③ Unilateral, first-time patients or patients with past seizures but no residual neurological dysfunction;
④ Clear consciousness, stable vital signs, stable condition;
(5) Be able to understand the instructions of the therapist or instrument, and be able to complete the required actions;
⑥ Simple three-level balance evaluation method of sitting position two and above.

排除标准:

①溶栓治疗及脑出血手术;
②病情恶化出现新的梗死或出血,重度脑水肿或昏迷等情况;
③既往运动功能障碍;
④除脑卒中外的其他神经系统疾病;
⑤重要脏器功能减退或衰竭;
⑥失语或认知功能障碍;
⑦并发恶性肿瘤

Exclusion criteria:

① Thrombolysis and intracerebral hemorrhage surgery;
② New infarction or hemorrhage, severe cerebral edema or coma, etc.;
③ previous motor dysfunction;
(4) neurological diseases other than stroke;
(5) Functional decline or failure of important organs;
⑥ aphasia or cognitive dysfunction;
⑦ Complicated malignant tumor
