研究目的：

通过颈动脉血流测定来监测大脑的血流、术中监测脑饱和度、S100β蛋白及MMSE评分等指标的变化，来进一步探索控制性降压下脊柱手术患者大脑的血流和患者术后认知功能的关系和影响，为临床安全使用控制性降压这一技术提供理论依据。  

Objectives of Study：

Carotid blood flow measurement was used to monitor brain blood flow, intraoperative monitoring of brain saturation, S100β protein and MMSE score and other indexes, so as to further explore the relationship and influence between blood flow in the brain of patients undergoing controlled blood pressure lowering and postoperative cognitive function, so as to provide a theoretical basis for the safe use of controlled blood pressure lowering technology.

药物成份或治疗方案详述：

硝酸甘油0.5μg/（kg·min）、艾司洛尔2.5μg/（kg·min）（硝酸甘油：艾司洛 尔为1:5），微量泵输注速度硝酸甘油0.5μg/（kg·min）为初始量进行控制性降压，并根据血压逐渐增加剂量使平均动脉压（MAP）降至基础值的70％～80％，从开始降压到达到稳定的目标血压在15min内完成，此后根据术中心率和血压调整硝酸甘油剂量。 

Description for medicine or protocol of treatment in detail：

Nitroglycerin 0.5μg/(kg·min), esmolol 2.5μg/(kg·min) (nitroglycerin: 1:5 for esloer), micropump infusion speed nitroglycerin 0.5μg/(kg·min) for the initial amount of controlled blood pressure, and gradually increase the dose according to blood pressure to reduce the average arterial pressure (MAP) to 70%~80% of the basal value, from the beginning of lowering blood pressure to reaching a stable target blood pressure within 15min, Thereafter, the dose of nitroglycerin is adjusted according to the central rate and blood pressure. 

纳入标准：

年龄＞18岁、ASA分级Ⅰ~Ⅱ级、受教育年限≥6年。

Inclusion criteria

Age> 18 years old, ASA grade I.~II., years of education≥ 6 years.

排除标准：

手术涉及椎体小于等于2个，资料不完善、急诊手术、术中联合其他手术或麻醉方式、伴有严重呼吸、循环系统基础疾病者、肝肾功能和凝血功能异常者、贫血者（血红蛋 白（HB）＜100 g/L 或血细胞比容（HCT）＜30％）、对局部麻醉药过敏、脑梗后遗症期、存在精神疾病史或长期服用影响神经功能药物者、长期饮酒者、严重视力或听力障碍者、酒精滥用史、无法完成简易智能精神状态检查量表 （MMSE）评分者、存在气管插管禁忌者、术中循环波动剧烈或手术时间＞4 h者。

Exclusion criteria：

The operation involves less than or equal to 2 vertebral bodies, imperfect data, emergency surgery, intraoperative combined with other surgical or anesthesia methods, patients with severe respiratory, circulatory underlying diseases, abnormal liver and kidney function and coagulation function, anemia (blood red egg white (HB) <100 g/L or hematocrit (HCT) <30%), allergy to local anesthetics, sequelae of cerebral infarction, history of mental illness or long-term use of drugs affecting neurological function, long-term drinking, severe visual or hearing impairment, history of alcohol abuse, Those who are unable to complete the Simple Intelligent Mental State Examination (MMSE) scorer, those who have contraindications to endotracheal intubation, those who have severe intraoperative circulation fluctuations, or those whose operation time is > 4 hours.
