研究目的：

通过观察对比分析高危PDR患者PRP或PPV治疗后的视觉效果和病情发展情况以及患者的满意度，比较PPV与PRP在治疗高危PDR患者的临床疗效及安全性。  

Objectives of Study：

The clinical efficacy and safety of PPV and PRP in the treatment of high risk PDR patients were compared by observing and comparing the visual effects, disease development and the satisfaction of patients with high risk PDR after PRP or PPV.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.大于18岁的男性或者女性患者 2.被诊断为1型或者2型糖尿病 3.根据《我国糖尿病视网膜病变临床诊疗指南（2014年版）》评级，患者DR病情属于高危PDR，标准为：出现视网膜新生血管（NVE）或者视乳头新生血管（NVD），当NVD＞1/4-1/3视乳头直径（DA）或NVE＞1/2DA，或伴有视网膜前出血或者玻璃体出血。 4.最佳矫正视力＞0.05（34个字母） 5.既往无RPR治疗史 6.屈光介质清晰，瞳孔可以充分散大，以便PRP治疗以及获得更清晰的OCT图像和眼底彩色照相

Inclusion criteria

1. Aged＞18 years
2. Diagnosed as type 1 or type 2 diabetes
3. According to the rating of “the Clinical Diagnosis and Treatment Guidelines for diabetes Retinopathy in China ”(2014 Edition)，the patients are diagnosed as high risk PDR. The criteria includes : NVD>1/4-1/3 papillary diameter (DA) or NVE>1/2DA, or accompanied by pre retinal hemorrhage or vitreous hemorrhage.
4. Best corrected vision acuity＞0.05 (34 letters)
5. No treatment for PRP
6. The refractive medium is clear, and the pupil can be fully dilated to facilitate PRP treatment and obtain clearer OCT images and fundus color photography

排除标准：

1.血压>180/110 mmHg（1 kPa=7.5 mmHg）等血压控制不理想者 2.严重心功能、肝肾功能不全者 3.过去4个月内有严重的心脑血管疾病者 4.过去2个月内接受过抗VEGF治疗 5.过去4个月内有皮质类固醇治疗史（玻璃体腔内） 6.排除严重白内障、孔源性视网膜脱离、Eales病、视神经病变者 7.其他合并有青光眼、葡萄膜炎、视网膜血管阻塞者、活动性眼周或眼内炎症 8.研究眼有明显的玻璃体视网膜纤维血管增生、玻璃体积血或牵拉性视网膜脱离 9.研究眼存在虹膜或前房角新生血管形成，或新生血管性青光眼

Exclusion criteria：

1. Blood pressure>180/110 mmHg (1 kPa=7.5 mmHg). 2. Patients with severe cardiac, liver and kidney insufficiency 3. Patients with serious cardio-cerebrovascular diseases in the past 4 months 4. Have received anti-VEGF treatment in the past 2 months 5. Have received corticosteroid treatment in the past 4 months (intravitreal) 6. Patients with severe cataract, rhegmatogenous retinal detachment, Eales disease, and optic neuropathy 7. Patients with glaucoma, uveitis, retinal vascular obstruction, active periocular or intraocular inflammation 8. The study eyes are accompanied with obvious vitreoretinal fibrovascular hyperplasia, vitreous hemorrhage or tractive retinal detachment 9. The study eyes are accompanied with neovascularization of iris and anterior chamber angle or neovascular glaucoma
