研究目的：

本研究将通过临床实验研究方法，验证连续性高频振荡治疗对机械通气患者肺通气的影响，为其进一步推广应用提供科学可靠的理论研究基础。  

Objectives of Study：

This study will verify the effect of Continuous High-Frequency Oscillation Therapy on lung aeration in mechanically ventilated patients through clinical experimental research methods, and provide a scientific and reliable theoretical research basis for its further application.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.18-80岁；
2.GCS≤8分(言语反应为1分)；
3.满足下列条件：体温＜38.5℃,心率40-120次/分,收缩压90-180mmHg,和或舒张压≤110mmHg,平均动脉压≥65mmHg, ICP＜20mmHg,腹内压＜12mmHg,氧浓度FiO2＜60%，呼吸频率≤35次/分,血氧饱和度≥90%；
4.机械通气≥24h,且预计继续通气时间≥1周；
5.签署知情同意书

Inclusion criteria

1. 18 to 80 years;
2. GCS ≤ 8 points (verbal response is 1 point);
3. the following criteria are met: body temperature <38.5°C, heart rate 40-120 beats/min, systolic blood pressure 90-180 mmHg, and or diastolic blood pressure ≤110 mmHg, mean arterial pressure ≥65 mmHg, ICP <20 mmHg, intra-abdominal pressure <12 mmHg, oxygen concentration FiO2 <60%, respiratory rate ≤35 breaths/min, oxygen saturation ≥90%;
4. mechanical ventilation ≥ 24h, and the expected duration of continued ventilation ≥ 1 week;
5. Signed informed consent form

排除标准：

1.BMI≥40kg/m2；
2.严重脊柱、胸廓畸形；
3.植入起搏器者；
4.躁动无法配合者；
5.已知或疑似膈肌失能如：膈神经断裂、神经肌肉疾病等；
6.未处理的张力性气胸；
7.肺出血；
8.怀孕；
9.符合入选标准，无排除标准＞72h；
10.医疗团队认为其他不适合开展治疗的原因。

Exclusion criteria：

1. BMI ≥ 40 kg/m2;
2. severe spinal and thoracic deformities;
3. persons with implanted pacemakers;
4. those who are agitated and unable to cooperate;
5. known or suspected diaphragmatic dysfunction e.g. phrenic nerve rupture, neuromuscular disease, etc;
6. untreated tension pneumothorax;
7. pulmonary haemorrhage;
8. pregnancy;
9. meeting inclusion criteria without exclusion criteria >72h;
10. other reasons why the medical team considers it inappropriate to carry out the treatment.
