研究目的：

基于渥太华决策支持框架构建健康教育内容，提高CKD患者透析方式决策能力，减少决策困境，提高患者生活质量水平。  

Objectives of Study：

Based on the Ottawa decision support framework, health education content is constructed to improve the decision-making ability of dialysis methods in CKD patients, reduce decision-making difficulties, and improve the quality of life of patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合CKD诊断标准的4-5期患者，eGFR<30 ml·min-1·(1.73 m2)-1；
②年龄≧18周岁，生活能够自理或有照顾者；
③患者或者照顾者认知功能正常；
④志愿无偿参与本研究并签署知情同意书者。

Inclusion criteria

① Patients with stage 4-5 who meet the diagnostic criteria for CKD, eGFR<30 ml·min-1· (1.73 m2)-1
② Age≧ 18 years old, able to take care of themselves or have care;
③ The patient's or caregiver has normal cognitive function;
④ Those who voluntarily participate in this research and sign the informed consent form.

排除标准：

①已接受肾移植手术或已经开始接受透析治疗；
②因健康状况只能进行某一种特定透析治疗者；
③不能按时随访者。

Exclusion criteria：

① Have received kidney transplantation or have started dialysis treatment;
② Those who can only undergo a specific dialysis treatment due to their health conditions;
③ Those who cannot follow up on time.
