研究目的：

探究慢性腹泻患者群体当中，粪便钙卫蛋白筛查对器质性慢性腹泻的检出率，寻找合适的钙卫蛋白诊断阈值，为腹泻患者钙卫蛋白筛查提供循证依据。  

Objectives of Study：

To explore the detection rate of fecal calprotectin screening in patients with chronic diarrhea for organic chronic diarrhea, and to find appropriate threshold for the diagnosis of calprotectin, so as to provide evidence-based basis for the screening of calprotectin in patients with diarrhea.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

慢性腹泻受试者：
1）男女不限，年龄≥18周岁；
2）腹泻时长≥3个月，同时伴有大便频率≥2次/天，或大便性状为糊状/水样（布里斯托大便分型6、7型）；
3）拟接受结肠镜/病理活检检查；
4）同意参加本次临床研究，并签署知情同意书。
健康对照：
1）男女不限，年龄≥18周岁；性别、年龄和BMI与病例组匹配；
2）无腹泻症状，健康状况良好，目前无消化系统疾病，且无严重消化系统疾病史或其他严重疾病史；
3）同意参加本次临床研究，并签署知情同意书。

Inclusion criteria

Subjects with chronic diarrhea:
1) Male and female, ≥18 years old;
2) Duration of diarrhea ≥3 months, accompanied by stool frequency ≥2 times/day, or the stool character is mushy/watery (Bristol stools type 6, 7);
3) To undergo colonoscopy/pathological biopsy;
4) Agree to participate in this clinical study and sign informed consent.

Healthy control:

1) Male and female, ≥18 years old; Gender, age, and BMI were matched with the case group;
2) No symptoms of diarrhea, good health condition, no current digestive disease, and no history of severe digestive disease or other serious diseases;
3) Agree to participate in the clinical study and sign the informed consent.

排除标准：

1）腹水、腹部包块或者有腹部/盆腔手术史，如胆胰手术（接受过阑尾切除术或剖宫产术后未发生肠梗阻等肠道并发症的患者；接受过肠息肉内镜治疗的患者不排除）；
2）有下列疾病史：弥漫性小肠疾病、糖尿病植物神经病变、甲亢相关腹泻、类癌综合征、嗜铬细胞瘤、阿狄森病、严重系统性疾病（免疫系统疾病不排除）；
3）腹泻与长期或近期服用泻药相关；
4）已知或怀疑胃肠道梗阻、狭窄、瘘管或活动性出血及其他不能肠镜检测的情况；
5）妊娠期或哺乳期女性；
6）精神疾病患者（无法配合研究或不能签署知情同意）；
7）三个月内参加过其他临床试验或正在参加其他临床试验的受试者；
8）研究人员认为存在任何其他不适合受试者参与研究的影响因素。

Exclusion criteria：

1) Ascites, abdominal mass, or history of abdominal/pelvic surgery, such as biliopancreatic surgery (patients who have received appendectomy or cesarean section without intestinal complications such as intestinal obstruction; Patients who had received endoscopic treatment for intestinal polyps were not excluded);
2) A history of the following diseases: diffuse small bowel disease, diabetic phytoneuropathy, hyperthyroid-related diarrhea, carcinoid syndrome, pheochromocytoma, Addison's disease, serious systemic diseases (immune system diseases are not excluded);
3) Diarrhea is associated with long-term or recent use of laxatives;
4) Known or suspected gastrointestinal obstruction, stenosis, fistula or active bleeding, and other conditions that cannot be colonoscopically detected;
5) Pregnant or lactating women;
6) Patients with mental illness (unable to cooperate with research or unable to sign informed consent);
7) Subjects who have participated in or are participating in other clinical trials within three months;
8) The researcher considers that there are any other influencing factors that may not be appropriate for the subject to participate in the study.

诊断试验：

Diagnostic Tests：

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

本研究需要诊断的疾病是慢性腹泻的器质性/功能性病因。因此采用症状+病史+肠镜检查+组织活检活检+其他可选择的特殊检查的方式来确定器质性腹泻的诊断。对于功能性腹泻采用罗马IV标准分型。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The disease to be diagnosed in this study is the organic/functional cause of chronic diarrhea. Therefore, the diagnosis of organic diarrhea is determined by symptom + history + colonoscopy + tissue biopsy + other optional special tests. Rome IV standard classification was used for functional diarrhea.

1.粪便钙卫蛋白浓度； 2.黏膜损伤血清学指标：二胺氧化酶；乳酸；细菌内毒素；血清Zonulin蛋白。

Index test:

1. Fecal calcaretin concentration; 2. Serological indexes of mucosal injury: Diamine oxidase; Lactic acid; Bacterial endotoxin; Serum Zonulin protein.

目标人群（可以是某种疾病患者或正常人群，详细描述其疾病特征，注意应纳入符合分布特点的全序列病例，具有良好的代表性）

1.器质性病因导致的慢性腹泻患者； 2.功能性病因导致的慢性腹泻患者； 3.肠道健康人群。

Target condition (The target condition is a particular disease or disease stage that the index test will be intended to identify. Please specify the characteristics in detail; the population should has a complete spectrum and good representative):

1. Patients with chronic diarrhea due to organic causes; 2. Patients with chronic diarrhea due to functional etiology; 3. People with healthy intestines.

容易混淆的疾病人群（即与目标疾病不易区分的一种或多种不同疾病，应避免采用正常人群对照的病例-对照设计）：

1.药物引起的慢性腹泻患者； 2.罕见病因引起的慢性腹泻患者，罕见病因包括：弥漫性小肠疾病、糖尿病植物神经病变、类癌综合征、嗜铬细胞瘤、阿狄森病、严重系统性疾病； 3.慢性腹泻患者，但是不需要进行病因筛查的人群：腹水、腹部包块或者有腹部/盆腔手术史，如胆胰手术（接受过阑尾切除术的患者；剖宫产术后未发生肠梗阻等肠道并发症的患者。

Population with condition difficult to distinguish from the target condition, the normal population in a case-control study design should be avoid:

1. Patients with chronic diarrhea caused by drugs; 2. Patients with chronic diarrhea caused by rare causes, including diffuse small bowel disease, diabetic phytoneuropathy, carcinoid syndrome, pheochromocytoma, Addison's disease, and severe systemic disease; 3. Patients with chronic diarrhea who do not require etiological screen.
