研究目的:

炎症性肠病(IBD)是一种发生在胃肠道的慢性炎症,包括溃疡性结肠炎(UC)和克罗恩病(CD)。IBD的病因尚未完全确定,大量证据表明其发病机制涉及环境、遗传、免疫和微生物因素。肠易激综合征 (IBS) 是一种脑-肠相互作用疾病,其特征是在没有胃肠道 (GI) 炎症的情况下长期反复腹痛和排便习惯改变,全球患病率约为11.2%。 这两种肠道疾病通常表现出焦虑和抑郁的共病症状,并且症状发作通常是由心理社会压力引发的,这表明这两种疾病都存在脑-肠轴调节异常。由于IBD及IBS的反复发作和复杂的演变,其诊断和治疗较为困难,目前临床对IBD及IBS均无高效疗法。胃肠道可通过迷走神经等传入神经支配,迷走神经与消化道疾病关系密切。经皮耳迷走神经刺激(taVNS)是一种新兴的非侵入性刺激迷走神经的神经调节疗法,其通过向耳朵施加低强度脉冲电流刺激迷走神经耳支,是一种易操作、副作用小且耐受性良好的电刺激治疗方法。经过二十多年的研究,taVNS可治疗多种疾病,其中包括功能性消化不良等功能性胃肠道疾病。根据taVNS的作用原理,推测其有望用于治疗IBD及IBS,但相关研究报道较少。 近年来,应用磁共振成像(MRI)方法研究功能性胃肠道疾病的中枢机制已成为主要研究方向之一。有学者通过MRI研究发现IBD及IBS患者存在脑网络异常。因此,本项目将围绕“taVNS可高效调节IBD及IBS患者异常的脑网络”的研究假说,利用多模态MRI技术采集taVNS治疗IBD及IBS患者前后的脑影像数据,应用VBM、FreeSurfer、独立成分分析、Graph、Granger、jMRUI等数据分析方法,结合临床疗效评估数据,分析重要脑区的自发活动、功能连接、神经递质含量变化及结构改变等结果,从结构-功能-代谢层面观察tVNS治疗对IBD及IBS患者的中枢调节作用,探索其中枢调节机制。  

Objectives of Study:

Inflammatory bowel disease(IBD)is a chronic inflammatory process occurring in the gastrointestinal tract,including ulcerative colitis(UC)and Crohn's disease(CD).The etiology of IBD has not been fully established, and there is substantial evidence that its pathogenesis involves environmental, genetic, immunologic, and microbiological factors.Irritable bowel syndrome (IBS) is a brain-gut interaction disorder characterized by prolonged recurrent abdominal pain and altered bowel habits in the absence of gastrointestinal (GI) inflammation, with a global prevalence of 11.2%.Both bowel disorders commonly exhibit comorbid symptoms of anxiety and depression, and episodes of symptoms are often triggered by psychosocial stress, suggesting abnormal brain-gut axis regulation in both disorders.Due to the recurrence and complexity of IBD and IBS, diagnosis and treatment are difficult, and there are no effective clinical treatments for IBD and IBS.The gastrointestinal tract can be innervated by afferent nerves such as the vagus nerve, which is closely related to digestive tract diseases.Transcutaneous oto迷走神经 stimulation(taVNS)is an emerging neuromodulatory noninvasive stimulation of the vagus nerve that stimulates the auricular branches of the vagus nerve by applying low-intensity pulsing currents to the ear,an easy-to-use,small-side-effect,and well-tolerated electrostimulation therapy.After more than two decades of research, taVNS has been shown to treat a variety of disorders, including functional gastrointestinal disorders such as dyspnoea.Based on the principles of action of taVNS, it has been speculated to be useful in the treatment of IBD and IBS, but few studies have been published. In recent years, the use of magnetic resonance imaging (MRI) methods to investigate the central mechanisms of functional gastrointestinal diseases has become one of the major research directions.Network abnormalities have been found in IBD and IBS patients by MRI studies.Therefore, this project will focus on the hypothesis that taVNS can efficiently regulate abnormal brain networks in patients with IBD and IBS. We will use multimodal MRI to collect brain imaging data before and after taVNS treatment in patients with IBD and IBS. Using VBM, FreeSurfer, independent component analysis, Graph, Gramer, jMRUI and other data analysis methods, we will combine clinical efficacy assessment data, analyze the results of spontaneous activity, functional connectivity, neurotransmitter content changes and structural changes in important brain regions.

纳入标准:

a.年龄18~65岁;
b.符合炎症性肠病及肠易激综合征诊断标准;
c.右利手;
d.研究前一周能够停用治疗胃肠道疾病的药物。

Inclusion criteria

a. age 18 to 65 years
b. meet diagnostic criteria for inflammatory bowel disease and irritable bowel syndrome;
c. right hand;
d. Drugs that can be discontinued for gastrointestinal disorders in the first week of the study.

排除标准:

a.有严重的合并症(如不受控制的高血压或胃肠道出血、严重的心肺脑疾病等)及冠心病等;
b.影响胃肠动力的自身免疫性疾病,如硬皮病、皮肌炎;
c.胃肠道手术史;
d.任何级别的肠癌、胃的器质性病变如消化性溃疡、胃肿瘤;
e.存在肠结核等非炎症性肠病等感染性病变;
f.存在其他的功能性胃肠病;
g.在招募前 1 周内服用治疗胃肠道疾病药物、影响胃肠动力的药物或无法戒掉这些药物;
h.不能配合完成相应评估及检查而中途退出者;
i.磁共振检查禁忌症(如:有不能取下的金属假牙或者其他金属植入物,幽闭恐惧症等)。

Exclusion criteria:

a. Severe comorbidities (e.g., uncontrolled hypertension or gastrointestinal bleeding, severe cardiopulmonary and encephalopathy) and coronary heart disease;
b. autoimmune diseases affecting gastrointestinal motility, such as scleroderma and dermatomyositis;
c. history of gastrointestinal surgery;
d. Organic lesions of the stomach and intestines of any grade such as peptic ulcers and gastric tumors;
e. presence of infectious lesions such as non-inflammatory bowel diseases such as enteric tuberculosis;
f. Other functional gastrointestinal disorders are present;
g. taking drugs for gastrointestinal disorders, drugs that affect GI motility, or drugs that cannot be withdrawn within 1 week before recruitment;
h. unable to cooperate with the appropriate assessment and examination;
i. Contraindications to magnetic resonance imaging (e.g., failure to remove metal dentures or other metal implants, claustrophobia, etc.).
