研究目的：

1. 探讨α-骨化醇在防治儿童肾病综合征激素相关骨病上的有效性； 2. 探讨α-骨化醇在使用糖皮质激素的肾病综合征患儿中是否可以提高尿蛋白的缓解率。  

Objectives of Study：

1. To explore the effectiveness of alfacalciitol in the prevention and treatment of hormone-related bone diseases in children with nephrotic syndromewhether; 2. To explore whether alfacalcidol can improve the remission rate of urine protein in children with nephrotic syndrome who use glucocorticoids.

药物成份或治疗方案详述：

治疗组为维D钙咀嚼片1片/天（碳酸钙300mg+VitD 100U）+α-骨化醇0.03μg/kg/天（20公斤以上儿童1μg/天）；对照组为维D钙咀嚼片1片/天（碳酸钙300mg+VitD 100U）。 

Description for medicine or protocol of treatment in detail：

Treatment group: vitamin D calcium masticatory tablets 1 tablet/day (calcium carbonate 300mg+VitD 100U) + α-calcitol 0.03μg/kg/ day (1μg/ day for children over 20 kg); Control group: vitamin D calcium chewable tablets 1 tablet/day (calcium carbonate 300mg+VitD 100U). 

纳入标准：

1.年龄1-18周岁男性或女性受试者，需知情同意；
2.确诊原发性肾病综合征需全身性糖皮质激素治疗；
3.筛选入组前6个月无使用糖皮质激素药物史；
4.筛选入组前6个月无使用钙剂、维生素D及维生素D衍生物药物史；
5.既往无骨折病史；
6.筛选入组前双能X线骨密度测定腰椎及桡骨密度Z评分均≥-1.0SD；
7.筛选入组前血钙及血磷均正常范围；
8.筛选入组前身高HtSDS≥-2.0；
9.肝肾功能正常患儿；
10.血压正常患儿。

Inclusion criteria

1. Male or female subjects at the age of 1 to 18 years, informed consent is required;
2. The diagnosis of primary nephrotic syndrome requires systemic glucocorticoid therapy;
3. No history of glucocorticoid use within 6 months before screening;
4. No history of using calcium, vitamin D and vitamin D derivatives within the 6 months before screening;
5. No history of fracture;
6. Before screening, dual-energy X-ray bone mineral density determination of lumbar spine and radius bone mineral density Z score >= -1.0SD;
7. Normal range of blood calcium and blood phosphorus before screening;
8. Height HtSDS >= -2.0 before screening;
9. Children with normal liver and kidney function;
10. Children with normal blood pressure.

排除标准：

1. 恶性肿瘤史；
2. 甲状腺功能亢进或减退；
3. 甲状旁腺功能亢进或减退；
4. 先天性肌营养不良；
5. 除GC所导致以外的原发或继发性骨丢失性疾病如慢性肾功能不全、生长激素缺乏症、佝偻病、肾性佝偻病、范可尼综合征、肾小管酸中毒及其他内分泌代谢性疾病或肾小管性疾病；
6. 已知对钙或维生素D补充剂不耐受；
7. 活动性感染或感染史（定义为在筛查前4周内使用全身抗感染药物的任何活动性感染）；
8. 严重感染（定义为在筛查前8周内需要住院治疗或静脉注射抗感染药物）；
9. 热量-蛋白质营养不良。

Exclusion criteria：

1. Refractory hypercalcemia
2. Refractory hypertension
3. Progressive decline in liver and kidney function
4. Severe infection
5. Complicated with malignant tumors, abnormal thyroid function and abnormal parathyroid function
6. Adverse reactions to the drug intolerance
7. People undergoing renal replacement therapy due to medical conditions
