研究目的：

评价改良舌侧入路与传统颊侧入路在拔除下颌中低位近中/水平阻生第三磨牙的手术中手术持续时间、术后出血、疼痛、感染、干槽症等指标的差异。  

Objectives of Study：

To evaluate the clinical effects of modified lingual approach and traditional buccal approach in the extraction of mid and low mandibular impacted third molars．

药物成份或治疗方案详述：

每名患者分两次拔除两侧M3M，所有手术均由同一外科医生完成，保证在拔除对侧M3M之前，初始手术一侧的术后反应可以完全消退。 1) 术前常规预防性用药。 2) 患者用抗菌洗必泰漱口水漱口1 min，采用计算机控制的局部麻醉给药系统，用1.7ml Primacaine Adrenaline (1/100000)进行下颌神经阻滞麻醉和M3M牙周浸润。 3) 对照侧术式：采用颊侧翻瓣，于M2M颊侧龈沟内切开，绕过远中颊角，再从远中面向磨牙后区偏颊侧切开约1.5厘米到下颌支，剥离抬起颊侧粘骨膜瓣，避免翻瓣范围过大，特别避免翻起舌侧组织；小心牵拉保护组织瓣，使用高速外科手机圆形钻头在M3M牙冠颊侧远中去骨，充分冲水冷却，显露M3M牙冠颊侧最大径轮廓和釉-牙骨质界（CEJ），注意控制去骨范围，避免去除颊侧外斜嵴；沿釉牙骨质界用裂钻分切，分开牙冠牙根，然后将牙冠进一步切割成舌侧和颊两半，分别去除，使牙根冠方有足够的脱位空间，使用牙挺向近中、向上挺出牙根，如有必要，对多根牙在根分叉处进一步分切，分别去除各牙根。 4) 试验侧术式：采用舌侧翻瓣，切口从M2M的远中颊角龈缘开始，在外斜嵴内侧向远中延伸约1.5cm到下颌支，在 M3M 牙冠和磨牙后区骨表面仔细剥离抬起舌侧黏骨膜组织，显露舌侧骨板边缘，形成无张力的舌侧瓣，颊侧软组织不翻瓣；用骨膜剥离器插入骨与软组织之间轻柔牵开组织瓣，为舌神经提供保护，使用球钻在牙冠表面和舌侧骨板内侧去骨，显露牙冠舌侧边缘和釉牙骨质界(CEJ)，舌侧去骨深度约1-2mm，注意避免穿透舌侧骨板；斜行分切牙冠，从舌侧CEJ根方开始向颊侧牙冠最大径冠方使用裂钻分牙，将斜型分切的牙冠进一步分切成舌侧和颊侧半，分别去除，使牙根冠方、舌侧有足够脱位空间，使用牙挺楔入颊侧牙周间隙，向舌侧挺动，使牙根松动，如有必要，对多根牙在根分叉处进一步分切，分别去除各根。 5) 牙拔除后用可吸收明胶海绵填充牙槽窝，丝线缝合软组织。患者进行术后冷敷，保持口腔卫生，口服洛索洛芬钠0.6g每日3次作为镇痛和消炎药物，口服甲硝唑0.2g每日2次、头孢呋辛酯0.25g每日2次，连续3天。 

Description for medicine or protocol of treatment in detail：

M3M was extracted twice for each patient, and all operations were performed by the same surgeon to ensure that the postoperative reaction on the original surgical side could be completely resolved before the removal of the contralateral M3M. 

纳入标准：

1. 双侧下颌第三磨牙阻生类型、位置相同，均为近中/水平阻生；
2. 阻生M3M最高部分低于M2M咬合面；
3. 双侧上颌非阻生萌出，或缺如；
4. 自愿签署知情同意书。

Inclusion criteria

1. The type and position of impacted mandibular third molars on both sides are the same, both mesial/horizontal impacted;
2. The highest part of the impacted M3M is lower than the occlusal surface of the M2M;
3. Bilateral maxillary non-impacted eruption, or absence;
4. Voluntarily sign the informed consent form.

排除标准：

1. 术前有感染或肿胀迹象，或术前疼痛程度在视觉模拟量表 (VAS) 上大于1；
2. 怀孕及备孕；
3. 使用抗凝药物；
4. 患有可能影响伤口愈合的疾病，如未控制或治疗不佳的糖尿病、放疗和/或化疗史、血液疾病和免疫抑制等；
5. 每天吸烟超过5支的患者。

Exclusion criteria：

1. There are signs of infection or swelling before operation, or the degree of preoperative pain is greater than 1 on the visual analog scale (VAS);
2. Pregnancy and pregnancy preparation;
3. Use anticoagulant drugs;
4. Suffering from diseases that may affect wound healing, such as uncontrolled or poorly treated diabetes, history of radiotherapy and/or chemotherapy, blood diseases and immunosuppression, etc.;
5. Patients who smoke more than 5 cigarettes per day.
