研究目的：

本研究拟通过在老年髋关节置换手术麻醉中使用瑞马唑仑和丙泊酚，比较其对术中循环，术后不良反应及术后苏醒情况的影响。确定瑞马唑仑的安全性和有效性，为危重病人的全身麻醉提供更多的参考和选择。  

Objectives of Study：

The purpose of this study was to compare the effects of remimazolam and propofol on intraoperative circulation, postoperative adverse reactions, and postoperative recovery. To determine the safety and effectiveness of remimazolam, to provide more references and choices for general anesthesia in elderly and critical patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. X线或CT影像检查明确存在髋关节破坏；符合中华学会骨科分会确定的髋关节置换手术指征（①保守治疗无效的髋关节骨关节炎，②关节功能明显受限，影响生活，③多种原因所致股骨头坏死出现股骨头塌陷，④老年人移位股骨颈骨折，内固定手术失败的股骨颈骨折）；
2. 首次接受髋关节置换手术；
3. 具备自主沟通能力；
4. 年龄≥60岁。

Inclusion criteria

1. Presence of hip destruction confirmed by X-ray or CT imaging; conforming to indications for hip replacement determined by Chinese Medical Association Orthopaedic Society:
(1) Ineffective conservative treatment of hip osteoarthritis;
(2) Obviously limited joint function affecting life;
(3) Femoral head collapse in femoral head necrosis due to a variety of reasons;
(4) Displaced femoral neck fractures in the elderly and femoral neck fractures with failed internal fixation);
2. Hip replacement for the first time;
3. Ability to communicate independently;
4. Aged >= 60 years.

排除标准：

1. 合并心、脑、肾、造血功能障碍；
2. 既往存在严重精神疾病或谵妄状态；
3. 合并老年痴呆症或癫痫史；
4. 既往存在冠心病或高血压、糖尿病等。

Exclusion criteria：

1. Involved heart, brain, kidney and hematopoietic dysfunctions;
2. Previous history of severe mental illness or delirium;
3. History of Alzheimer s disease or epilepsy;
4. Previous history of coronary heart disease, hypertension or diabetes mellitus.
