研究目的：

评价芪参益气滴丸治疗冠心病心衰的临床疗效及安全性  

Objectives of Study：

To evaluate the clinical efficacy and safety of Qishen Yiqi Dripping Pills on ischemic heart failure

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄40~79岁；
（2）符合冠心病心衰诊断标准：
①LVEF≤45%（改良Simpson法）；
②有陈旧心肌梗死病史，经或未经血运重建（PCI或CABG）治疗；或冠状动脉造影或冠脉CTA提示冠状动脉至少一支主要分支管腔直径狭窄在50%以上，经或未经血运重建治疗，研究者认为该冠脉病变与患者心力衰竭的关系密切；
③有或无呼吸困难、乏力和液体潴留（水肿）等。
（3）心衰病史或临床出现心衰症状3个月以上；
（4）NYHA心功能Ⅱ~Ⅳ级；
（5）签署知情同意书。

Inclusion criteria

1. Age 40 to 79;
2. Patients with ischemic heart failure:
(1) LVEF less than or equal to 45% measured by echocardiography in modified Simpson method;
(2) History of prior myocardial infarction with or without percutaneous coronary intervention (PCI) or/and coronary artery bypass grafting (CABG);
(3) Coronary angiography (CAG) or coronary CTA shows more than or equal to 50% stenosis in at least one of the main coronary arteries with or without revascularization, of which the researcher thinks that it is closely related to heart failure;
(4) With or without dyspnea, fatigue and fluid retention (edema) etc.;
(5) History of heart failure or present with heart failure symptoms for at least 3 months;
(6) New York Heart Association (NYHA) Class II to IV;
(7) Submitted informed consent.

排除标准：

1. 急性心力衰竭或慢性心力衰竭急性加重；。
2. 有下列疾病之一者：①急性冠脉综合征30天内；②血运重建6个月内；③药物难以控制的高血压（收缩压≥180mmHg和/或舒张压≥110mmHg）；④Ⅱ度Ⅱ型以上窦房或房室传导阻滞未置入起搏器治疗、难以控制的恶性心律失常；⑤扩张型心肌病；⑥肥厚梗阻性心肌病；⑦心肌炎；⑧肺动脉栓塞；⑨严重瓣膜疾病；⑩肺心病； ○11近6个月内脑卒中者等。
3. 心脏再同步化治疗者。
4. 近7天内静脉使用利尿、强心、扩血管药物。
5. 甲亢等严重内分泌疾病者。
6. 恶性肿瘤患者。
7.血红蛋白≤9g/dl。
8.谷丙转氨酶＞正常上限2倍ULN（正常值上限）者；
9.血清肌酐＞265μmol/L者；.
10.精神病患者。
11.妊娠或计划妊娠者，哺乳期妇女。
12.对试验药物可疑或明确过敏。
13.近两月内参加其他研究者。
14.因肢体障碍或其他原因不能进行6分钟步行试验者。

Exclusion criteria：

1. Acute heart failure or acute exacerbation of chronic heart failure;
2. Those who have one of the following diseases:
3. Acute coronary syndrome within 30 days;
4. Revascularization therapy within 6 months;
5. Uncontrolled hypertension with systolic pressure more than or equal to 180mmHg and/or diastolic pressure more than or equal to 110mmHg;
6. Second degree type 2 or worse sinoatrial /atrioventricular block without implantation of pacemaker or uncontrolled malignant cardiac arrhythmia;
7. Dilated cardiomyopathy ;
8. Hypertrophic obstructive cardiomyopathy;
9. Myocarditis;
10. Pulmonary artery embolism;
11. Severe valvular heart disease;
12. Pulmonary heart disease;
13. Stroke within 6 months;
14. Cardiac resynchronization therapy;
15. Applied diuretics, cardiotonic agents or vasodilators intravenously within 7 days;
16. Severe endocrine diseases such as hyperthyroidism, etc.
17. History of malignant tumor;
18. Hemoglobin (HB)<=9g/dl;
19. Alanine aminotransferase (ALT) > 2 times the upper limit of normal;
20. Serum creatinine > 265umol/L;
21. Mental disorder;
22. Being pregnant, planning for pregnancy or breastfeeding;
23. Suspicious or definite allergy to intervention drugs;
24. Participated in other trials within 2 months;
25. Unable to walk autonomously due to physical disabilities.
