研究目的：

本项目聚焦12-18岁青少年高角型突面畸形患者，运用自主知识产权的H20自锁直丝弓固定矫治器系统，通过压低、扩展、逆旋上牙弓，前导、逆旋下颌，刺激髁突及下颌生长，促进下颌生长改良式“延展”，显著增加颏部突显度，增进面部侧貌美，扩大上气道，提升患者社交自信心理。项目拟采用与协作单位共同制定的在国内外处领先地位的创新优化矫治方案，规范化培养30名中青年医师掌握创新优化矫治方案，进而通过实用性随机对照研究，对90例青少年高角突面畸形患者进行综合诊治，比较创新优化矫治方案与传统方案对青少年高角型突面畸形患者的有效性和安全性。  

Objectives of Study：

This project focuses on adolescents with high-angle dentofacial protrusion from 12 to 18 years old, using the H20 self-ligating fixed appliance system with independent intellectual property rights. By depressing, extending, rotating the upper dentition, and counterclockwisely rotating the mandible, the growth of condyle and mandible are stimulated and promoted. Advancement of mandible will significantly increase the prominence of the chin, enhance the facial profile, expand the upper airway, and enhance the patients' social self-confidence. The project intends to adopt the innovative and optimized treatment protocol jointly developed with the cooperative unit. 30 young and middle-aged orthodontists will be trained to master the innovative and optimized treatment protocol. 90 adolescents with high-angle dentofacial protrusion will be enrolled and randomized to receive either innovative and optimized correction schemes or traditional schemes. The effectiveness and safety will be compared between the two groups.

纳入标准：

1.年龄12-18岁，性别不限；
2.突面畸形，颏部形态差，下颌后缩，ANB>5°, 上颌骨发育基本正常；
3.垂直生长型为高角倾向的儿童，SN-MP>35°；
4.受试者依从性良好，能按要求配合矫治并定期复查。

Inclusion criteria

1. Aged 12-18 years, no gender limit;
2. Protrusion deformity, poor chin morphology, mandibular retrusion, ANB>5 degree, maxillary development is basically normal;
3. Children with long-face growth pattern, SN-MP>35 degree;
4. The subjects are required good compliance and regular follow-up.

排除标准：

1.有颞下颌关节紊乱症状者；
2.金属过敏患者；
3.因患有全身疾病不适合正畸治疗；
4.下颌矢状向发育不足，经专家组审核需要进行功能矫治的患者；
5.严重垂直向发育过度，经专家组审核必须采取垂直向控制措施的患者。

Exclusion criteria：

1. Patients with temporomandibular disorders;
2. Patients with metal allergy;
3. Unsuitable for orthodontic treatment due to systemic disease;
4. Patients with sagittal mandibular deficiency requiring mandibular advancement treatment, after review by the expert group;
5. Patients with severe vertical overgrowth who must take vertical control measures after review by the expert group.
