研究目的：

主要目的： 1.建立并优化完善具有华西特色、学科联盟适用的H2H全程营养管理模式，助力成果转为华西特色、中国特色证据，为国家指南、共识、规划教材及国民营养健康决策奠基，为全国医疗机构规范化全程营养管理提供参考； 2.建立并完善智慧医疗思想指导下的在线在位、上下联动、多媒体健康传播、智能个体化定制的H2H营养管理生态圈；完善学科联盟内医、技、护、管多学科云端协作机制，推进H2H营养管理广泛落地。  

Objectives of Study：

Main purpose: 1. Establish and optimize the H2H whole-process nutrition management model with West China characteristics and applicable to the discipline alliance, and help the results to be translated into evidence with West China characteristics and Chinese characteristics, laying a foundation for national guidelines, consensus, planning textbooks and national nutrition and health decision-making, and providing reference for the standardization of whole-process nutrition management in medical institutions across the country. 2. Establish and improve H2H nutrition management ecosystem under the guidance of smart medical thought, which includes online in-place, up-down interaction, multimedia health communication and intelligent personalized customization; Improve the multidisciplinary cloud collaboration mechanism of medicine, technology, nursing and management within the discipline alliance, and promote the extensive implementation of H2H nutrition management.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.经病理检查确诊为肿瘤患者；
2.18岁及以上；
3.能够进行有效的沟通交流完成测评；
4.计划接受化疗或者放疗的患者；
5.自愿参与本研究。

Inclusion criteria

1. Patients diagnosed with tumor by pathological examination;
2. Aged >=18 years;
3. Be able to communicate effectively to complete the assessment;
4. Patients who plan to receive chemotherapy or radiotherapy;
5. Voluntary participation in this research.

排除标准：

1.合并其他原发肿瘤者；
2.放弃治疗自动出院者；
3.接受姑息治疗者。

Exclusion criteria：

1. Combined with other primary tumors;
2. Those who give up treatment are automatically discharged from hospital;
3. Those receiving palliative care.
