研究目的：

1.探究超声测量下腔静脉直径及塌陷指数，快速评估老年患者术前血容量的准确性及相关性； 2.验证超声测量下腔静脉塌陷指数预测血容量指导输液，降低腰硬联合麻醉后低血压的价值。  

Objectives of Study：

1. to explore the accuracy and correlation of ultrasonic measurement of inferior vena cava diameter and collapse index in elderly patients with preoperative blood volume; 2. to verify the value of ultrasonic measurement of inferior vena cava collapse index in predicting blood volume and reducing hypotension after combined spinal and epidural anesthesia.

药物成份或治疗方案详述：

第一部分：根据麻醉后是否发生低血压分为低血压组和非低血压组，纳入193例患者超声测量仰卧位IVC 直径及塌陷指数； 第二部分：纳入大于第一部分试验dIVC-CI取值范围下限值老年患者60人，由随机数字表法分为对照组( n=30）、试验组（n=30）。 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

择期全麻或腰硬联合麻醉手术患者；年龄65-89岁；美国麻醉医师学会（ASA）分级Ⅰ～Ⅱ级；BMI 18～25 kg／m2；病人本人及家属知情并签署知情同意书；患者有意识、有自主呼吸；患者自愿接受测量；患者测量部位没有纱布或绷带覆盖

Inclusion criteria

Patients undergoing elective anesthesia surgery; aged 65-89 years old; American Academy of anesthesiologists (ASA) grade I ~ II; BMI 18 <=25 kg/m2; patients and their families knew and signed informed consent; patients were conscious and breathing autonomously; patients volunteered to be measured; patients were not covered with gauze or bandages.

排除标准：

急诊手术患者；周围血管疾病、严重的心血疾病、不稳定性心绞痛或射血分数低于40％的患者;表现为呼吸窘迫、腹内压升高、预期气道或精神功能障碍的患者；自主神经系统疾病的患者；植入式起搏器或心律转复器的患者；下腔静脉疾病，服用血管紧张素转化酶抑制剂或血管紧张素受体拮抗剂的高血压患者；麻醉前基础SBP<90 mmHg；术前突发紧急生命体征变化、危及生命的患者；腰麻禁忌

Exclusion criteria：

Emergency surgical patients;Peripheral vascular disease, severe cardiovascular disease, unstable angina pectoris or ejection fraction less than 40%; Patients with respiratory distress, increased abdominal pressure, expected airway or mental dysfunction; Patients with autonomic nervous system disease; Patients with implantable pacemakers or rhythm converters; Lower vena vena disease, hypertension patients taking angiotensin conversion enzyme inhibitors or angiotensin receptor antagonists; Preanesthetic basic SBP<90mmHg;Sudden change of vital signs before operation, life-threatening patients; Waist anesthesia taboos.

存在血容量不足，给予250ml羟乙基淀粉，10 min输注完毕。再次测量ICV-CI，直至补液达到正常，之后按照“四二一补液法”补液

For patient with insufficiency of blood volume, 250ml hydroxyethyl starch was given and transfused in 10 minutes. Measure ICV-CI again until the rehydration is normal, and then rehydrate according to '421 rehydration method'
