研究目的：

肾积水是儿童泌尿系统最常见的先天畸形之一。目前，临床上对于婴儿肾积水的干预策略缺乏共识。本研究拟采用多中心、前瞻性、随机对照的方式，评估目前临床中常用的干预时机与方式对患儿肾功能的保护效力，为婴儿肾积水的干预时机和方式判断提供高级别临床证据，从而使更多的患儿能够得到最佳的诊疗方案。  

Objectives of Study：

Hydronephrosis is the most common congenital abnormities. There is still no consensus on the intervene strategy. This prospective, multi-center, randomized, controlled trial aim to evaluate the effect of clinical major intervene strategy for protecting renal function, so that could provide high level clinical evidence for a better intervene scheme.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.2022年1月-2024年12月就诊于研究相关医疗中心的年龄≤3月龄的肾积水患儿。同时满足如下条件的进入重度肾积水A组别：
(1)超声评估25mm≤APD＜35mm；
(2)SFU分级为II级或III级（超声下肾盂扩张，肾盏可见或扩张）；
(3)肾动态显像评估患肾功能≥40%；
2.满足如下条件之一的进入重度肾积水组别：
(1)超声评估APD≥35mm；
(2)SFU分级为IV级（肾实质变薄）；
(3)肾动态显像评估患肾功能＜40%。

Inclusion criteria

1. Children with hydronephrosis aged <=3 months who visited the relevant medical center from January 2022 to December 2024. At the same time meet the following conditions to enter the severe hydronephrosis group A:
(1) Ultrasound assessment 25mm<=APD<35mm;
(2) SFU grade is II or III (renal pelvis dilation under ultrasound, renal calyces visible or dilated);
(3) Renal dynamic imaging assessment of affected renal function >= 40%;
2. Those who meet one of the following conditions enter the severe hydronephrosis group:
(1) Ultrasound assessment APD>=35mm;
(2) SFU grade is grade IV (thinning of renal parenchyma);
(3) The renal function assessed by dynamic renal imaging is less than 40%.

排除标准：

1. 存在后尿道瓣膜等下尿路梗阻性疾病；
2. 肾脏发育畸形的，如马蹄肾、重复肾、异位融合肾等；
3. 双侧肾积水；
4. 下腔静脉后输尿管等因素导致的肾积水；
5. 原发性膀胱输尿管返流、膀胱输尿管连接部梗阻、梗阻性巨输尿管、输尿管开口囊肿等疾病；
6. 合并其他系统严重畸形的患儿；
7. 就诊时已在外院接受过造瘘或置管等干预性治疗的；
8. 依从性差或不具备随访条件的患者。

Exclusion criteria：

1. The presence of lower urinary tract obstructive diseases such as posterior urethral valves;
2. Malformed kidneys, such as horseshoe kidney, duplication kidney, ectopic fusion kidney, etc.;
3. Bilateral hydronephrosis;
4. Hydronephrosis caused by factors such as posterior inferior vena cava and ureter;
5. Diseases such as primary vesicoureteral reflux, vesicoureteral junction obstruction, obstructive megaureter, ureteral orifice cyst;
6. Children with severe malformations of other systems;
7. Those who have received interventional treatments such as ostomy or catheter placement in other hospitals when visiting a doctor;
8. Patients with poor compliance or without follow-up conditions.

对重度肾积水A组和重度肾积水B组各自160例患儿，由研究者采用区组随机化的方法按照1:1的比例分别随机分入两种不同处理单元。区组长度为4，结合随机数字表中取随机数字确定患者组别。区组随机化软件为R(4.0版）。
