研究目的：

观察对比GLP-1RA周制剂司美格鲁肽与二甲双胍治疗超重或肥胖PCOS患者的疗效  

Objectives of Study：

To compare the efficacy of semaglutide and metformin in the treatment of overweight or obese PCOS patients

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1)符合中国标准诊断的PCOS
2)年龄在18-45岁；
3)身体指数大于等于24 kg/m2；
4)受试者知情并签署知情同意书。

Inclusion criteria

1）PCOS as diagnosed by Chinese criteria
2）BMI ≥ 24 kg/m2
3）Age 18-45 years
4）Informed written consent

排除标准：

1)诊断为甲状腺髓样癌(MTC)或多发性内分泌瘤综合征2型(MEN2)，或者有上述疾病家族史，内分泌科医生筛查时发现有甲状腺髓样癌或甲状腺结节家族史；
2)使用已知影响胰岛素敏感性的药物:二甲双胍(筛查前3个月内不能使用)、10天内口服糖皮质激素、非典型抗精神病药物、免疫抑制剂、HIV药物、激素避孕(筛查前6个月内不能使用)，皮肤贴剂或阴道环避孕方法。减肥药物或兴奋剂。使用含有其他GLP-1激动剂的其他产品；
3)正在怀孕或哺乳的妇女。在研究期间怀孕的发展将需要退出研究；
4)病情严重，需在60天内住院治疗；
5)糖尿病患者（根据WHO1999年糖尿病诊断标准）；
6)年龄小于18岁或大于45岁；
7)身体指数小于24kg/m2；
8)贫血，血红蛋白小于110g/L；
9)被诊断为限制知情同意的重大精神疾病或发育障碍；
10)正在使用降压药物；
11)已知任何严重的肾脏疾病或筛查时eGFR <45ml/min/1.73m2；
12)有严重胃肠道疾病史(如胃轻瘫)；
13)有胆结石病史；
14)有未经治疗的甲状腺疾病；
15)对研究药物的任何成分过敏: 司美格鲁肽，二水磷酸二钠，丙二醇，苯酚，盐酸和氢氧化钠；
16)有其他原因的高雄激素(例如:肿瘤，先天性肾上腺皮质增生症)或闭经(未经治疗的甲状腺疾病，肿瘤，原发性卵巢衰竭，泌乳素瘤)；
17)处于活跃期或正在治疗的神经性厌食症或暴饮暴食症。

Exclusion criteria：

1）Diagnosed with or have a family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Family history of medullary thyroid cancer or thyroid nodule palpated by endocrinologist at screening.
2）Use of medications known to affect insulin sensitivity: metformin (cannot have been used in the 3 months prior to screening), oral glucocorticoids within 10 days, atypical antipsychotics, immunosuppressant agents, HIV medications, hormonal contraception (cannot have been used in the 6 months prior to screening). Dermal patch or vaginal ring contraception methods.Weight loss medications or stimulants. Use of other products containing other GLP-1 agonists.
3）Currently pregnant or breastfeeding women. Development of pregnancy during the study period will necessitate withdrawal from the study.
4）Severe illness requiring hospitalization within 60 days.
5）Diabetes as by The World Health Organization(WHO, 1999) criteria.
6）Age ＜18 or ＞45 years.
7) BMI ＜24kg/m2.
8）Anemia, defined as Hemoglobin < 110 g/L.
9) Diagnosed major psychiatric or developmental disorder limiting informed consent.
10）Use of blood pressure medications.
11）Known renal disease of any severity or an eGFR at screening of <45ml/min/1.73m2.
12) History of severe GI disease (e.g. gastroparesis).
13) History of gallstones
14) Untreated thyroid disease
15) Allergy to any of the ingredients of the study medication: semaglutide, disodium phosphate dihydrate, propylene glycol, phenol, hydrochloric acid and sodium hydroxide.
16) Other causes of hyperandrogenism (example: tumor, Congenital adrenal hyperplasia) or amenorrhea (untreated thyroid disease, tumor, primary ovarian failure, prolactinoma).
17）Active symptoms or undergoing treatment for anorexia nervosa or binging/purging disorder.

随机分组将由海南医学院第一附属医院内分泌科进行，内分泌科将使用计算机化的随机数生成器，使用随机大小的排列块生成随机化时间表。参与者将以1:1的比例随机分配到二甲双胍或司美格鲁肽组。

The randomization will be carried out by the Department of Endocrinology, The First Affiliated Hospital of Hainan Medical University. The Department of Endocrinology will use a computerized random number generator to generate a randomization schedule using random-sized perarray blocks. Participants will be randomly assigned
