研究目的：

探索先天性心脏病术后低氧性呼吸衰竭患儿实施肺复张的安全性和有效性。  

Objectives of Study：

To explore the safety and efficacy of recruitment maneuver in children with hypoxic respiratory failure after congenital heart disease.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 解剖完全矫治,无心内分流的心脏外科术后机械通气病人；
2. 年龄:28天到14岁；
3. 氧合指数PaO2/FiO2＜300mmHg。

Inclusion criteria

1. Mechanical ventilation after cardiac surgery with complete anatomical correction and no intraventricular shunt;
2. Age: 28 days to 14 years;
3. Oxygen index PaO2/FiO2 < 300mmHg.

排除标准：

1. 血流动力学不稳定(去甲肾上腺素＞0.2ug/kg/min或肾上腺素＞0.2ug/kg/min，或顽固性代谢性酸中毒)；
2. 气管插管不带气囊；
3. 气胸，未安置引流管；
4. 支气管胸膜瘘；
5. 颅内压增高(＞20mmHg)；
6. 临床医生认为不合适(例如肺动脉高压危象，右心功能差等)；
7. 家属不同意。

Exclusion criteria：

1. Hemodynamic instability (defined as need for norepinephrine > 0.2 ug / kg/min or epinephrine > 0.2 ug / kg/min, or refractory metabolic acidosis);
2. Endotracheal tube without cuff;
3. Pneumothorax without a chest tube in place;
4. Bronchopleural fistula;
5. Increased intracranial pressure( > 20mmHg);
6. The clinician team deem the patient unacceptable candidate (such as pulmonary hypertensive crisis, poor right ventricular function);
7. Failure to sign informed consent.
