研究目的：

评价全程引导下静态导板和动态导航应用于口腔上前牙即刻种植手术中对种植体植入精度的影响  

Objectives of Study：

To evaluate the effect of fully guided static template and dynamic navigation system on the accuracy for implant placement in immediate dental implant surgery

药物成份或治疗方案详述：

纳入受试者并分组后，按照以下流程进行操作： 1.术前对受试者进行口内检查，面扫（放松面相、微笑面相、大笑面相），口扫，CBCT拍摄，DSD设计，mock up，数字化种植方案设计。 2.试验组（动态导航组）：根据受试者术区情况在导航软件内利用术前CBCT、口扫信息、理想修复体信息进行数字化种植设计，术前进行手机定位器和参考板的标定，术区和术前设计CBCT进行配准，常规术前服用抗生素、消毒、局麻、翻瓣后，微创拔除无法保留的患牙后，充分搔刮拔牙窝，生理盐水冲洗清除炎性组织，备洞后在拔牙窝内植入种植体。若需要进行同期骨增量，则于跳跃间隙填充颗粒状同种异体骨混合自体骨屑，表面覆盖Bio-gide胶原膜。旋上愈合帽或临时修复体，减张缝合。对照组（静态导板组）：根据受试者术区情况进行数字化种植设计，设计观察窗、固位钉套筒，生成数字化手术导板数据，打印制作导板，送冷消留手术备用。常规术前服用抗生素、消毒术区、局麻、翻瓣后，确认导板完全就位后，植入固位钉，在导板引导下植入种植体。若需要进行同期骨增量，则于跳跃间隙填充颗粒状同种异体骨混合自体骨屑，表面覆盖Bio-gide胶原膜。旋上愈合帽或临时修复体，减张缝合； 3.术后即刻拍摄CBCT, 对完成种植体植入精度测量、统计分析的研究人员设盲； 4.术后两周拆线； 5.若未同期进行骨增量手术，则术后三月复查、二期手术以及后续最终修复； 6.若同期进行骨增量手术，则术后六月复查、二期手术以及后续最终修复； 

Description for medicine or protocol of treatment in detail：

1. Preoperative oral examination, face scan (relaxed face, smiling face, laughing face), intra-oral scan, CBCT, DSD design, mock up and digital implant design are performed on the subjects. 2. Experimental group (dynamic navigation group) : Preoperative calibration and registration procedure is completed. Tooth extraction and implant surgery are performed under the guidance of dynamic navigation system guidance; Control group (static template group) : Preoperative fixation of static template in patient's mouth is completed. Tooth extraction and implant surgery are performed under the guidance of static template guidance; 3. CBCT is taken immediately after surgery, and researchers who complete the measurement and statistical analysis of implant implantation accuracy were blinded; 4. Stitches are removed two weeks after surgery; 5. If bone augmentation surgery is not performed at the same time, after three months subsequent restoration will be performed; 6. If bone augmentation surgery is performed at the same time, six months after the surgery, restoration procedure will be performed; 

纳入标准：

1)年龄在18周岁-60周岁之间（含18周岁和60周岁），性别不限；
2)上颌前牙美学区存在广泛龋坏, 无法行牙体牙髓治疗的牙、有少量骨丧失的外伤性缺失牙、根折需拔除的患牙、严重牙周骨质丧失, 无溢脓的牙周病牙、牙齿内吸收或外吸收、正畸后牙根吸收需拔除、周围软组织健康, 有足够的黏骨膜可以利用的患牙；
3)拔牙位点无急性炎症或脓性渗出，能够完成炎症清除；
4)即刻种植位点需要有足够的骨量，拔牙位点根方骨壁足以维持种植体的初期稳定性；
5)缺损部位为有利型骨缺损，包括植体唇侧火山口样骨缺损以及植体暴露表面位于骨弓轮廓之内的开窗式和开裂式骨缺损；
6)无全身疾病种植手术禁忌证；
7)受试者充分了解本试验的受益和风险，愿意参与并签署知情同意书。

Inclusion criteria

1) Aged between 18 and 60 (including 18 and 60), regardless of gender;
2) the maxillary anterior teeth aesthetic area cannot be preserved;
3) Inflammation can not be removed;
4) The immediate implant site should have enough bone mass to maintain the initial stability of the implant;
5) The defect site was favorable type;
6) No contraindications of systemic diseases;
7) The patients are informed the benefits and risks of this study and is willing to participate and sign the informed consent.

排除标准：

1)上颌前牙美学区根尖急性感染的患牙、急性炎症活动期的牙周病牙、软组织急性炎症的患牙；
2)拔牙位点存在急性感染、溢脓肿胀、未愈合的慢性根尖周炎，无法做到彻底清创；
3)广泛的根方骨缺损，不足以维持种植体的初期稳定性；
4)有不良口腔卫生习惯不能纠正者
5)咬合关系严重不良者或严重习惯性磨牙症患者;
6)心脏疾病（即II级和II级以上心功能疾病）；肝、肾功能异常（ALT、AST ＞正常值1.5倍；糖尿病患者，血糖水平控制不佳者（经控制空腹血糖仍≥8.0 mmol/L）；
7)未得到控制的新陈代谢疾病，如糖尿病、软骨病、甲状腺疾病、严重肝肾功能障碍；
8)骨质疏松症、骨软化症及骨硬化症等骨疾病患者；
9)恶性肿瘤、活动性感染性病变、代谢性骨病活动期产生的各种骨缺损；
10）身感染或手术部位局部急性感染者；
11)再生障碍性贫血等血液性疾病，近3个月服用抗凝药物或凝血检查不在正常范围内者；
12)使用可能影响软硬组织愈合的药物，如长期使用镇静催眠药（连续使用3个月以上）、长期使用非甾体类消炎药或肾上腺皮质激素（连续使用3个月以上）、正在或近3个月内曾接受可能影响或促进骨代谢的药物治疗的患者；
13)在过去6个月内以及骨愈合期间，吸烟严重的患者（吸烟超过20支/日）；
14)自身免疫性疾病；
15)正在进行放射治疗；
16)精神疾病、药物滥用、酗酒；
17)已知对产品所用材料过敏者；
18)妊娠期及哺乳期妇女。

Exclusion criteria：

1) Affected teeth with acute apical infection in the aesthetic area, acute inflammatory periodontal disease, and affected teeth with acute soft tissue inflammation;
2) Acute infection, abscess swelling, chronic unhealed periapical periodontitis exist at the tooth extraction site, and inflammation can not be removed;
3) Bone volumes are insufficient to maintain the initial stability of implants;
4) Those who have bad oral hygiene habits that cannot be corrected
5) Patients with serious bad occlusal relationship or severe habitual bruxism;
6) Heart disease (i.e. grade II and above heart function disease); Abnormal liver and renal function (ALT, AST > 1.5 times normal value; Patients with diabetes and poor blood glucose control (fasting blood glucose ≥ 8.0mmol /L after control);
7) Uncontrolled metabolic diseases, such as diabetes, rickets, thyroid disease, severe liver and kidney dysfunction;
8) Patients with bone diseases such as osteoporosis, osteomalacia and osteosclerosis;
9) Various bone defects in the active stage of malignant tumors, active infectious lesions and metabolic bone diseases;
10) Local acute infection of body infection or surgical site;
11) Patients with aplastic anemia and other blood diseases, who have taken anticoagulant drugs in the last 3 months or whose coagulation test is not within the normal range;
12) The use of drugs that may affect the healing of soft and hard tissues, such as long-term use of sedative-hypnotic drugs (continuous use for more than 3 months), long-term use of non-steroidal anti-inflammatory drugs or adrenal corticosteroids (continuous use for more than 3 months), or patients who have received drugs that may affect or promote bone metabolism in the past 3 months;
13) Heavy smokers (more than 20 cigarettes per day) in the past 6 months and during bone healing;
14) Autoimmune diseases;
15) Undergoing radiation therapy;
16) Mental illness, substance abuse, alcoholism;
17) People who are known to be allergic to the materials used in the product;
18) Pregnant and lactating women.

随机分组方案的产生：采用区组随机化设计，将进入研究的时间相近的4位受试者分为一个区组，根据进入研究的时间对受试者进行编号。由特定的人员在纳入受试者前生成随机分组序列，该人员仅参与随机分组方案生成和方案隐藏过程，不参与后续的临床研究。该人员利用EXCEL程序生成6组随机数字，每组4个随机数字，数字随机范围为0-100，数值较大的2个为试验组，较小的两个为对照组（例：第一区组中生成随机数字为66、32、1、78，则第一组1号和4号受试者进入试验组，3号和4号受试者进入对照组），若存在随机数字相同的情况，则再次生成随机数字。 随机分组方案的隐藏，将分组方案编号，并放入密封、不透光的信封中。每纳入完一个区组的受试者后，才能打开该区组的信封，按信封内的分组方案进行分组。
