研究目的：

开展老年共病患者衰弱影响相关预后研究，包括：1）基于老年共病数据库及生物样本库，整合临床表型、基因表型、共病指数、衰弱评估、认知功能、焦虑抑郁和生活质量等，利用机器学习算法和数据挖掘技术，分析影响老年衰弱患者预后的重要危险因素；2）根据不同危险因素制定个体化的精准诊疗策略，为老年共病衰弱患者打造诊断、评估、干预、监测为一体的精准管理体系，建立维护老年衰弱患者生活质量的临床诊疗优化方案，达到最优化管理的目的。  

Objectives of Study：

We will perfrom the research of the impact of frailty on the prognosis of elderly patients with comorbidity including: 1) Integrate clinical phenotype, gene phenotype, comorbidity index, frailty assessment, cognitive function, anxiety and depression and quality of life, and analyze the important risk factors affecting the prognosis of elderly patients with frailty by using machine learning algorithm and data mining technology; 2) Formulate individualized accurate diagnosis and treatment strategies according to different risk factors and create an accurate management system integrating diagnosis, evaluation, intervention and monitoring for elderly patients with comorbidity and weakness and eventually establish a clinical diagnosis and treatment optimization scheme to maintain the quality of the life of elderly patients with weakness.

纳入标准：

1.年龄≥65岁；
2.老年共病患者。

Inclusion criteria

1.Age >= 65 years;
2.Elderly comorbid patients.

排除标准：

1.不愿意参加该研究；
2.不能或不愿意配合基线或随访调查者；
3.研究人员认为不合适参加临床试验者。

Exclusion criteria：

1.Unwilling to participate in the study;
2.Those who are unable or unwilling to cooperate with the baseline or follow-up survey;
3.The researcher considers it inappropriate to participate in the clinical trial.
