研究目的：

1.建立广西具有心血管疾病高危因素和（或）具有心血管疾病的人群的标准化管理流程，获得这类患者筛查和确诊的纳入排除标准； 2.与现有管理流程相比较，包括各国指南与中国专家共识推荐的心血管疾病高危因素和/或心血管疾病人群的管理模式，探索适合广西心血管疾病高危因素和/或心血管疾病人群的管理模式。  

Objectives of Study：

1. Establish standardized management procedures for populations with high risk factors and/or cardiovascular diseases in Guangxi, and obtain inclusion and exclusion criteria for screening and diagnosis of such patients; 2. Compared with the existing management process, including the management mode of cardiovascular disease risk factors and/or cardiovascular disease population recommended by national guidelines and consensus of Chinese experts, to explore the management mode suitable for cardiovascular disease risk factors and/or cardiovascular disease population in Guangxi.

纳入标准：

年龄>18岁，存在心血管疾病高危因素和/或合并心血管疾病的所有患者。

Inclusion criteria

All patients > 18 years of age with cardiovascular disease risk factors and/or with cardiovascular disease.

排除标准：

1.合并肾功能失代偿期及以上的肾功能不全患者（CKD分期）；
2.合并肿瘤患者；
3.孕妇。

Exclusion criteria：

1. Patients with renal insufficiency combined with renal decompensation stage or above (CKD stage);
2. Combined tumor patients;
3. Pregnant women.
