研究目的：

1.分析不同程度认知功能障碍患者口腔微生态的特点，明确阿尔茨海默病与牙周炎的相关性； 2.通过外泌体多组学分析，探索在 AD 进程中起关键作用的分子标志物，为AD的早期诊断和早期治疗提供科学依据。  

Objectives of Study：

1. To analyze the characteristics of oral microecology in patients with cognitive impairment, and clarify the correlation between Alzheimer's disease and periodontitis; 2. To explore the molecular markers that play a key role in the process of AD through exosome multiomics analysis, so as to provide scientific basis for early diagnosis and treatment of AD.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.阿尔茨海默病入组标准：在满足National Institute on Aging–Alzheimer’s Association (NIA-AA)：
(1)主诉记忆减退，且有知情人证实；
(2)根据临床检查，可作出痴呆诊断，通过认知量表测试验证(20项ADL>26分；CDI>1分)；
(3)无意识障碍；
(4)全部病例均行头颅MRI检查以及相关实验室检查除外可导致记忆力和认知功能进行性损害的其他疾病；
(5)淀粉样蛋白/p-Tau在PET示踪中增加或脑脊液Aβ1-42增加伴/或P-Tau增加。
2.轻度认知障碍入组标准：
(1)以记忆力减退为主诉，且由知情人所证实；
(2)客观检查发现有与年龄和受教育水平不相称的轻度认知损害；
(3)总体认知功能正常，日常生活基本能力正常，复杂的日常或社会功能可轻微损害；
(4)没有痴呆(CDR—O．5分，20项ADL<26分)；
(5)筛选1年内无感染、梗塞或其他局灶性脑损伤的证据，也无涉及神经疾病的提示性临床症状；一个非关键性脑区的腔隙性病变且不被认为影响了研究对象的认知功能可包括在本研究中；
(6)有足够可配合完成神经心理学测试的视觉和听觉分辨力；
(7)病程在3个月以上。
3.认知功能正常组入组标准：
(1)简易精神状态检查表 (MMSE) >27/30分和正常神经心理评估；
(2)临床痴呆评分(CDR)得分= 0；
(3)无神经或精神疾病史；
(4)无记忆抱怨或认知缺陷。

Inclusion criteria

1. Alzheimer's disease inclusion criteria: After meeting the National Institute on Aging–Alzheimer's Association (NIA-AA):
(1) The chief complaint of memory loss, which is confirmed by an insider;
(2) According to clinical examination, the diagnosis of dementia can be made and verified by cognitive scale test (20 items ADL>26 points; CDI>1 point);
(3) unconscious disorder;
(4) Brain MRI examination and related laboratory examinations were performed in all cases, except for other diseases that could lead to progressive impairment of memory and cognitive function;
(5) Amyloid/p-Tau increased in PET tracing or CSF Aβ1-42 increased with/or increased P-Tau.
2. Inclusion criteria for mild cognitive impairment:
(1) The main complaint is memory loss, which is confirmed by an insider;
(2) Mild cognitive impairment disproportionate to age and education level is found by objective examination;
(3) The overall cognitive function is normal, the basic ability of daily life is normal, and the complex daily or social functions may be slightly impaired;
(4) No dementia (CDR-0.5 points, 20 items ADL< 26 points);
(5) No evidence of infection, infarction, or other focal brain injury within 1 year of screening, and no suggestive clinical symptoms involving neurological disease; lacunar lesions in a non-critical brain region and are not considered to have affected the study subjects of cognitive functions can be included in this study;
(6) have sufficient visual and auditory acuity to complete neuropsychological tests;
(7) The course of the disease is more than 3 months.
3. Inclusion criteria for normal cognitive function group:
(1) Mini-Mental State Examination (MMSE) > 27/30 and normal neuropsychological assessment;
(2) Clinical Dementia Rating (CDR) score = 0;
(3) No history of neurological or psychiatric diseases;
(4) No memory complaints or cognitive deficits.

排除标准：

1.急性感染；
2.CT/MR不耐受；
3.神经障碍；
4.头部受伤；
5.自身免疫性和系统性炎症性疾病（如系统性红斑狼疮、类风湿性关节炎或克罗恩病）；
6.酗酒或滥用药物。近期接受皮质激素或非甾体抗炎药（NSAID）或苯二氮卓类药物治疗。

Exclusion criteria：

1. Acute infection;
2. CT/MR intolerance;
3. Neurological disorders;
4. Head injury;
5. Autoimmune and systemic inflammatory diseases (such as systemic lupus erythematosus, rheumatoid arthritis or Crohn's disease);
6. Alcoholism or drug abuse. Recent treatment with corticosteroids or nonsteroidal anti-inflammatory drugs (NSAIDs) or benzodiazepines.
