研究目的：

针对氯胺酮预防苏醒期谵妄尚存在争议的地方，主要在于不同的给药时机可能对预防苏醒期谵妄的效果不一致，在这方面有待进一步的研究。氯胺酮因其镇静镇痛效果较好、及给药方便等优势，上世纪小儿麻醉中应用十分普遍，但因氯胺酮诸多不良反应和副作用，使其在临床上的应用逐渐被取代，而艾司氯胺酮是氯胺酮的右旋异构体，两者均通过作用于NMDA受体和阿片受体发挥麻醉与镇痛作用，但艾司氯胺酮与上述受体的亲和力是氯胺酮的2倍，且生物利用度更高、代谢更快，因此其镇痛作用显著，药物消除更快、苏醒更快。两者药物有着相似的药理学特点，同时又克服了氯胺酮的缺点，具有效能更高、可控性更强、副作用更少，不添加防腐剂，精神神经症状、呼吸道分泌物少，因此在临床中尤其是小儿麻醉时有更大的应用前景。既往研究证实艾司氯胺酮除有较强的麻醉作用外，亦有减少术后镇痛药用量、延长术后镇痛时间和预防阿片类药物导致的痛觉过敏等作用。本试验欲进一步研究使用艾司氯胺酮应用于小儿泌尿外科手术麻醉效果评价。 There is still controversy about the prevention of emergence delirium with ketamine, mainly because different timing of administration may have inconsistent effects on the prevention of emergence delirium, and further research is needed in this regard. Ketamine was widely used in pediatric anesthesia in the last century due to its good sedative and analgesic effect and convenient administration. It is the dextroisomer of ketamine, both of which exert anesthetic and analgesic effects by acting on NMDA receptors and opioid receptors, but esketamine has twice the affinity for the above receptors than ketamine, and has better bioavailability. High, metabolized faster, so its analgesic effect is significant, drug elimination is faster, and recovery is faster. The two drugs have similar pharmacological characteristics, and at the same time overcome the shortcomings of ketamine, with higher efficacy, stronger controllability, fewer side effects, no added preservatives, neuropsychiatric symptoms, and less respiratory secretions, so they are used in clinical practice. Especially in pediatric anesthesia, it has a greater application prospect. Previous studies have confirmed that esketamine not only has a strong anesthetic effect, but also reduces postoperative analgesic dosage, prolongs postoperative analgesic time, and prevents opioid-induced hyperalgesia. This trial intends to further study the use of esketamine in the evaluation of the anesthesia effect of pediatric urological surgery.  

Objectives of Study：

隐匿性阴茎是阴茎体位于会阴部深处的一种先天性外生殖器畸形外科矫治手术时间虽短但因阴茎体神经丰富且反应敏感手术可引起剧烈的疼痛,手术创伤和术后疼痛引起的应激反应可导致术后患儿生理功能紊乱和免疫功能抑制影响内环境稳定和术后康复,及时有效的术中及术后镇痛可减轻不良应激反应加快患者的免疫功能恢复和促进伤口愈合联合用药进行术中及术后镇痛,可减少镇痛药用量及不良反应又可抑制应激反应达到完善镇痛目的艾司氯胺酮是一种新型一种速效的非巴比妥类麻醉剂是氯胺酮中效价更强的右旋结构本研究观察艾司氯胺酮和瑞芬太尼单独或联合应用进行术中镇痛来评价艾司氯胺酮应用于全身麻醉下隐匿性阴茎矫治术中的效果。 Hidden penis is a congenital deformity of the external genitalia with the body of the penis located deep in the perineum. Although the operation time is short, surgery can cause severe pain due to the rich nerves of the body of the penis and the sensitivity of the operation. Stress reaction can lead to postoperative physiological function disorder and immune function suppression in children, affecting the stability of the internal environment and postoperative recovery, and timely and effective intraoperative and postoperative analgesia can reduce adverse stress reactions, accelerate patients' immune function recovery and promote wound healing. Combined medication for intraoperative and postoperative analgesia can reduce the amount of analgesics and adverse reactions, and can inhibit stress response to achieve the purpose of perfect analgesia. Esketamine is a new type of fast-acting non-barbiturate anesthetic, which is the more potent dextrorotatory structure of ketamine. This study observed esketamine and remifentanil alone or in combination for intraoperative analgesia to evaluate the effect of esketamine in concealed penile correction surgery under general anesthesia.

纳入标准：

择期行泌尿外科手术患儿60例，年龄 6-14岁，ASA 分级Ⅰ～Ⅱ级
A total of 60 children with elective urological surgery, aged 6-14 years old, with ASA grades I-II

选择择期行隐匿性阴茎矫治术的患者60例,年龄13岁,BMI13-30kg/m2,ASAⅠ级

排除标准：

①患儿家属拒绝者；②智力障碍及发育迟缓者；③有艾司氯胺酮使用禁忌症者；④合并其他重要器官疾患者。
① Those who are refused by the family members of the children; ② Those who have intellectual disability and developmental delay; ③ Those who have contraindications to the use of esketamine; ④ Those who have other vital organ diseases.

Exclusion criteria：

既往有系统性疾病(先心病中枢神经呼吸系统疾病及畸形等)者排除患有气道解剖畸形发育迟缓智力语言障碍肝肾疾病，麻醉药物过敏者
Those with previous systemic diseases (congenital heart disease, central nervous respiratory system diseases and malformations, etc.) are excluded from those with airway anatomical malformation, developmental delay, intellectual language disorder, liver and kidney disease, and allergy to anesthetic drugs
