了解社区血压标准化测量应用效果，为进一步推广应用奠定基础，最终实现基层医疗机构规范测量血压，提高居民血压知晓率、治疗率及控制率的目标。  

Objectives of Study：

To understand the effectiveness of standardized blood pressure measurement in communities and lay a foundation for further promotion and application. Finally, to promote process of standardized blood pressure measurement in primary medical institutions and improve residents' blood pressure awareness rate, treatment rate and control rate.

纳入标准：

1. 年龄在35-89岁之间；
2. 上海市社区慢性病管理系统中在管的高血压患者。

Inclusion criteria

1. Aged 35-89 years;
2. Hypertension patients recorded in Shanghai municipal management of chronic diseases system.

排除标准：

不愿配合本项目者。

Exclusion criteria：

Poor compliance.
