研究目的：

通过各临床病理指标的统计分析，探究中央组淋巴结清扫结合术中冰冻病理检查对甲状腺微小乳头状癌术中甲状腺切除及淋巴结清扫范围的指导作用，进而验证本方法的临床意义。  

Objectives of Study：

To investigate the clinical significance of central group of lymph node dissection combined with intraoperative frozen pathological examination, which can guide the surgical scope in papillary thyroid microcarcinoma.

药物成份或治疗方案详述：

甲状腺微小乳头状癌行甲状腺切除同时行患侧中央组淋巴结清扫（切除腺体前在单侧甲状腺腺体内注射纳米碳0.05ml，若需行双侧切除纳米碳注射总量控制在0.1ml以内）。在清除的淋巴脂肪组织中，由纳米碳的导引下选取3枚淋巴结送快速病理检查，根据病理结果确定患侧淋巴结清扫范围，具体手术方式有： 1. 单发微小乳头状癌无VI组淋巴结转移行患侧甲状腺切除+患侧中央组淋巴结清扫； 2. 单发微小乳头状癌伴VI组淋巴结转移行双侧甲状腺切除+双侧中央组淋巴结清扫，同时行患侧颈侧方淋巴结清扫（II、III、IV区）； 3. 单侧多发微小乳头状癌（术前超声及术中冰冻病理确认），先行双侧甲状腺切除+患侧中央组淋巴结清扫；若术中冰冻示患侧中央组淋巴结有转移行对侧中央组+患侧颈侧方淋巴结清扫（II、III、IV区）； 4. 双侧多发微小乳头状癌（术前超声及术中冰冻病理确认），行双侧甲状腺切除+双侧中央组淋巴结清扫；有中央组淋巴结转移者行中央组淋巴结转移侧的颈侧方淋巴结清扫（II、III、IV区）。 

Description for medicine or protocol of treatment in detail：

Thyroid micropapillary carcinoma was treated with thyroidectomy and lymph node dissection in the affected central group (0.05ml of carbon nanoparticles were injected into unilateral thyroid glands before gland resection, and the total amount of carbon nanoparticles injected was controlled within 0.1ml if bilateral resection was required). In the cleared lymphatic adipose tissue, 3 lymph nodes were selected and sent for rapid pathological examination under the guidance of carbon nanoparticles. According to the pathological results, the scope of lymph node dissection on the affected side was determined. The specific surgical methods were as follows: 1. Single micropapillary carcinoma without group VI lymph node metastasis underwent thyroidectomy of the affected side and lymph node dissection of the central group of the affected side; 2. Unilateral micropapillary carcinoma with group VI lymph node metastasis was treated with bilateral thyroidectomy + bilateral central group lymph node dissection, and lateral lymph node dissection of the affected neck (zone II, III, and IV) was also performed. 3. Unilateral multiple small papillary carcinoma (confirmed by preoperative ultrasound and intraoperative frozen pathology), bilateral thyroidectomy and central lymph node dissection were performed first; If intraoperative freezing showed metastatic lymph nodes in the central group of the affected side, the contralateral central group + lateral cervical lymph nodes of the affected side were dissected (zone II, III, IV). 4. Bilateral multiple small papillary carcinoma (confirmed by preoperative ultrasound and intraoperative frozen pathology), bilateral thyroidectomy and bilateral central lymph node dissection were performed; Patients with central lymph node metastasis underwent lateral cervical lymph node dissection (zone II, III, and IV) on the metastatic side of the central lymph node. 

纳入标准：

1. 年龄：18岁-75岁；
2. 术前有完整高分辨彩超记录肿瘤直径，且肿瘤最大径≤1cm；
3. 术前穿刺检查或术中冰冻病理确诊为甲状腺乳头状癌；
4. 初次行甲状腺癌手术治疗；
5. 入组前实验室检查结果符合手术条件，无手术禁忌症；
6. 患者自愿参加，依从性良好，并且签署知情同意书。

Inclusion criteria

1. Aged 18 to 75 years old;
2. The tumor diameter was completely recorded by high-resolution color ultrasound before surgery, and the maximum tumor diameter was <= 1cm;
3. Papillary thyroid carcinoma was confirmed by preoperative puncture examination or intraoperative freezing pathology;
4. Surgical treatment of thyroid cancer for the first time;
5. The results of laboratory examination before enrollment met the surgical conditions and there were no contraindications;
6. Patients participated voluntarily with good compliance and signed informed consent.

排除标准：

（1）心、脑、肺、肾功能不全、凝血功能异常等各种原因不能耐受手术者；
（2）有肿瘤远处转移（肺、骨等）；
（3）病人既往有颈部手术史或放射治疗史；
（4）孕妇、精神病患者；
（5）受试者的依从性差，不愿意参加者。

Exclusion criteria：

(1) Patients with heart, brain, lung, renal insufficiency, coagulation dysfunction and other reasons who cannot tolerate surgery;
(2) Distant metastasis of tumor (lung, bone, etc.);
(3) The patient has a history of previous neck surgery or radiotherapy;
(4) Pregnant women and mental patients;
(5) Patients have poor compliance and are unwilling to participate.
