研究目的：

本课题拟比较宫调音乐治疗PSD对IL-18、IL-1β和proBDNF/mBDNF指标的调节作用，五音宫调探索宫调音乐疗法可通过降低PSD患者IL-18、IL-1β和proBDNF/mBDNF指标，恢复其平衡状态，阐明音乐疗法通过调控炎症因子IL-18、IL-1β和proBDNF/mBDNF失衡而发挥抗抑郁作用的机制。  

Objectives of Study：

The purpose of this study is to compare the effects of GongDiao music on IL-18 and IL-1β And the regulation of proBDNF / mBDNF index, five tone palace tune exploration palace tune music therapy can reduce IL-18 and IL-1 in PSD patients β And proBDNF / mBDNF, restore its balance, and clarify that music therapy regulates inflammatory factors IL-18 and IL-1 β And the mechanism of antidepressant effect due to the imbalance of proBDNF / mBDNF.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.脑卒中诊断均符合1995年第4届全国脑血管病学术会议修订的诊断标准，均经头颅CT确诊；
2.抑郁诊断以《中国精神疾病分类与诊断标准第三版》(CCMD-3)抑郁症的诊断标准为主；
3.抑郁程度采用汉密顿抑郁量表（HAMD-17）量化评分，评分≥17分为中重度抑郁入组；
4.中医辨证分型：根据《中华人民共和国国家标准中医临床诊疗术语证候部分》、田德禄主编的《中医内科学》等可将抑郁症辨证为肝郁脾虚证，主症：忧愁善感、兴趣索然、食欲不振、神疲乏力、胁肋胀痛、时有太息、腹胀、寐欠佳。次症：胸闷、腹痛、痛泻、便溏。舌脉：舌质淡，舌苔白，脉弦细；
5.年龄在18-80岁之间；
6.初次诊断，且近2周未经过抗抑郁的药物治疗。

Inclusion criteria

1. The diagnosis of stroke was in line with the diagnostic criteria revised by the 4th National cerebrovascular disease academic conference in 1995, and all were diagnosed by cranial CT;
2. The diagnosis of depression is mainly based on the diagnostic criteria of depression in the third edition of Chinese classification and diagnostic criteria of mental diseases (CCMD-3);
3. The degree of depression was quantified by Hamilton Depression Scale (HAMD-17). If the score >= 17, it was classified as moderate and severe depression;
4. Syndrome differentiation of traditional Chinese medicine: according to the syndrome part of clinical diagnosis and treatment terms of traditional Chinese medicine in the national standard of the people's Republic of China and the internal medicine of traditional Chinese medicine edited by Tian Delu, depression can be differentiated into liver depression and spleen deficiency syndrome. The main symptoms are: sadness and interest, lack of interest, loss of appetite, mental fatigue, costal distension and pain, occasional excessive rest, abdominal distension and poor sleep. Secondary symptoms: chest tightness, abdominal pain, painful diarrhea, loose stool. Tongue pulse: light tongue, white tongue coating, fine pulse string;
5. Aged from 18 to 80 years;
6. The patient was diagnosed for the first time and did not receive antidepressant treatment in recent 2 weeks.

排除标准：

1.妊娠期、哺乳期妇女；
2.患有其它精神类疾病，如躁狂症、精神分裂症等；
3.目前患有严重呼吸、心血管、消化、血液系统等原发疾病者；
4.近期内患有严重感染或曾进行手术者；
5.脑卒中急性期。

Exclusion criteria：

1. Pregnant and lactating women;
2. Suffering from other mental disorders, such as mania, schizophrenia, etc;
3. Currently suffering from severe respiratory, cardiovascular, digestive, blood system and other primary diseases;
4. Those who have recently suffered from severe infection or have undergone surgery;
5. Acute stage of stroke.
