研究目的：

1. 探讨以氧供指数为导向的目标导向管理策略（GDP）（维持机体最低DO2i≥280mL/min/m2）能否降低急性A型主动脉夹层患者全弓置换术后AKI的发生率和改善其他临床结果。 2. 初步探讨GDP体外循环管理策略降低急性A型主动脉夹层患者全弓置换术后AKI的发生率的可能机制。  

Objectives of Study：

The aim of this trial was to determine whether a goal-directed perfusion (GDP) strategy aimed at maintaining oxygen delivery (DO2i) at ≥280mL/min/m2 reduce the incidence and improves clinical outcomes of patients with acute kidney injury (AKI) following surgery for acute type A aortic dissection (ATAAD) and to investigate the potential mechanism.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄≥18岁；
2. 诊断为急性A型主动脉夹层并接受体外循环下全弓置换手术的患者；
3. 美国麻醉协会分级Ⅰ-Ⅳ级；
4. 研究前签署了书面知情同意书。

Inclusion criteria

Patients must meet all the inclusion criteria:
1. aged ≥18 years;
2. undergoing Sun’s surgery under cardiopulmonary bypass (CPB) for patients with acute type A aortic dissection (ATAAD);
3. american Society of Anesthesiologists Ⅰ-Ⅳ level;
4. there is a written informed consent.

排除标准：

1. 术前患有慢性肾脏疾病的患者（接受肾脏替代治疗或eGFR （estimated glomerular filtration rate）<15mL/min/1.73m2）；
2. 贫血（术前HCT<30%）；
3. 围术期使用ECMO或VAD的患者；
4. 围术期心脏骤停；
5. 心脏移植；
6. 微创手术；
7. 孕妇；
8. 参加其他干预性研究者。

Exclusion criteria：

Patients who meet any of the following criteria will be excluded from participation:
1. chronic kidney disease (receipt of kidney replacement therapy or estimated glomerular filtration rate (eGFR）<15mL/min/1.73m2）;
2. anemia (preoperative Hct <30%);
3. perioperative treatment with extracorporeal membrane oxygenation (ECMO)or ventricular auxiliary device (VAD);
4. suffering perioperative cardiac arrest;
5. heart transplantation;
6. minimally invasive surgery;
7. pregnancy;
8. participating in other interventional studies.
