研究目的：

脑卒中以高发病率、高患病率、高致残率、高致死率为特点，严重影响人类的身体健康和心理健康，为当今社会带来了沉重的经济负担与发展负担。脑卒中后患者由于中枢神经系统受损，导致其运动或感觉传导系统发生改变，感觉输入异常、左右肢体间的协调性下降，进而影响到核心稳定性，因此会对后期平衡功能的改善产生负面影响。脑卒中后患者核心稳定性的下降经过医院内临床常规的康复治疗虽可取得一定的疗效，但在患者出院后由于康复措施的中断使得病情容易反复甚至恶化。但设计出以患者主动参与为特点的康复训练可解决这个困扰。因此，进一步探索简易高效、中西并重、传统与现代相结合的治疗方案，对脑卒中后期的康复具有十分重要的意义。本课题旨在通过观察吸气肌训练结合太极拳对脑卒中患者核心稳定性的影响，为其在脑卒中核心稳定性障碍患者康复治疗中的应用提供理论和实践依据  

Objectives of Study：

Stroke is characterized by high incidence, high prevalence, high disability rate and high mortality rate, which has seriously affected the human physical and mental health and brought heavy economic and development burden to today's society. The impaired central nervous system in patients after stroke leads to changes in their motor or sensory conduction systems, abnormal sensory input, and decreased coordination between left and right limbs, thereby affecting core stability, and thus negatively affecting the improvement of later balance function. The decline of core stability of patients after stroke has achieved certain efficacy after routine clinical rehabilitation in hospitals, but the condition is easily repeated or even worsened due to the interruption of rehabilitation measures after the patient was discharged. However, designing rehabilitation training characterized by active participation of patients can solve this puzzle. Therefore, it is of great significance to further explore the simple and efficient therapeutic scheme which pays equal attention to both Chinese and Western medicine as well as the combination of tradition and modernity for the rehabilitation in the late stage of stroke. The purpose of this study was to observe the effect of inspiratory muscle training combined with Tai Ji Chuan on core stability of patients with stroke, so as to provide a theoretical and practical basis for its application in the rehabilitation of patients with core stability disorders after stroke

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合缺血性脑卒中或脑出血的诊断标准，经颅脑CT和（或）MRI检查证实卒中后恢复期病人（即发病后6个月以内）②患者年龄45~80岁③患者生命体征平稳、生理稳定。存在自主呼吸④无明显感觉障碍、认知障碍、智力障碍，能很好配合评估。能够理解和主动配合呼吸肌训练的开展⑤家属及患者自愿配合治疗评估⑥下肢Brunnstrom分期达到Ⅳ期或Ⅳ期以上；下肢肌力达3级或3级以上；下肢肌张力不超过1级⑦FAC达2级或2级以上⑧存在呼吸肌肌力障碍

Inclusion criteria

① Patients who met the diagnostic criteria for ischemic stroke or cerebral hemorrhage, and the transcranial brain CT and/or MRI examinations confirmed that the patients were in the post-stroke recovery period (i.e., within six months after the onset); ② patients were aged from 45 to 80 years; ③ patients had stable vital signs and physiological stability. There was spontaneous breathing ④ There was no obvious sensory impairment, cognitive impairment and intellectual impairment, and assessment could be well coordinated. Ability to understand and actively cooperate with the development of respiratory muscle training ⑤ Evaluation of voluntary cooperation therapy by family members and patients ⑥ Brunnstrom stage of lower limb reaching stage IV or above; Lower limb muscle strength of 3 or more; Muscular tension of lower limbs shall not exceed grade 1 ⑦FAC reaches grade 2 or above ⑧ there is respiratory myotonia

排除标准：

①生命体征不稳定，血流动力学不稳定。②有严重心、肝、肾、肺等脏器疾病，存在严重的高血压、糖尿病和肠道疾病③先天性胸廓畸形及肋骨骨折病史或最近三个月进行过胸腹联合手术史④其它神经系统疾病导致平衡功能障碍者⑤呼吸系统疾病及其他神经系统疾病导致呼吸肌功能障碍⑥进行气管造口术，无自主呼吸的机械通气患者⑦严重认知障碍或意识障碍患者⑧严重关节病变，骨折愈合期⑨严重的脊柱后侧凸⑩存在静脉血栓

Exclusion criteria：

① Vital signs are unstable, and hemodynamics is unstable. ② Patients with severe heart, liver, kidney, lung and other organ diseases, severe hypertension, diabetes and intestinal diseases ③ patients with congenital thoracic deformity and rib fracture or patients with thoracoabdominal combined operation in the past three months ④ patients with balance dysfunction caused by other nervous system diseases ⑤ patients with respiratory muscle dysfunction caused by respiratory system diseases and other nervous system diseases ⑥ patients with tracheostomy and mechanical ventilation without spontaneous breathing ⑦ patients with severe cognitive impairment or consciousness disorder ⑧ patients with severe joint lesions, fracture healing stage ⑨ patients with severe kyphoscoliosis ⑨ patients with venous thrombosis

本试验拟将例符合纳入标准的脑卒中住院患者随机分成常规训练结合太极拳组和吸气肌训练结合太极拳组。首先生成随机数字表，然后按1∶1的比例随机分组，随机号由专人负责管理。
