研究目的：

本项目聚焦12-18岁青少年高角型突面畸形患者，运用自主知识产权的H20自锁直丝弓固定矫治器系统及改良下颌前伸类功能矫治器系统，通过压低、扩展、逆旋上牙弓，前导、逆旋下颌，刺激髁突及下颌生长，促进下颌生长改良式“延展”，显著增加颏部突显度，增进面部侧貌美，扩大上气道，强化唇舌肌功能训练，显著改善患者呼吸、语言、微笑等重要功能，提升患者社交自信心理。项目拟采用与协作单位共同制定的在国内外处领先地位的创新优化矫治方案，首先规范化培养30名中青年医师掌握创新优化矫治方案，进而通过实用性随机对照研究，对90例青少年高角突面畸形患者进行综合诊治，比较创新优化矫治方案与传统方案对青少年高角型突面畸形患者的有效性和安全性。不但在真实世界中评估培训效果，同时进行疗效机制分析，进一步细化、优化青少年疑难高角突面畸形的标准检查、诊断和矫治流程，并对此类患者的综合诊疗领先技术通过各种学习班及学术会议进行推广应用。  

Objectives of Study：

This project focuses on adolescents with high-angle protrusion from 12 to 18 years old, using the H20 self-ligating straight wire fixed appliance system with independent intellectual property rights and an improved mandibular extension appliance system to depress, extend, and rotate the upper teethreverse rotation of the mandible, stimulate the growth of condyle and mandible, and promote the growth of the mandible. Improved "extension" of mandible significantly increase the prominence of the chin, enhance the facial profile, expand the upper airway, strengthen the labioglossus functional training, and significantly improve the important functions of the patient's breathing, language, smile, etc., which could enhance the patient's social self-confidence. The project intends to adopt the innovative and optimized treatment plan jointly developed with the cooperative unit at home and abroad. Firstly, 30 young and middle-aged physicians will be trained to master the innovative and optimized treatment plan, and then through a practical randomized controlled study, 90 cases of adolescents with high-angle protrusion will be diagnosed and treated, comparing the effectiveness and safety of innovative and optimized correction schemes and traditional schemes for teenagers with high-angle protrusion. It not only evaluates the training effect in the real world, but also analyzes the curative effect mechanism to further refine and optimize the standard examination, diagnosis and correction procedures for difficult high-angle protrusion of young people, and to pass various learning classes on the leading technology of comprehensive diagnosis and treatment of such patients and academic conferences for promotion and application.

纳入标准：

①年龄12-18岁，性别不限；
②突面畸形，颏部形态差，下颌后缩，ANB>5?；
③上颌骨发育基本正常；
④垂直生长型为高角倾向的儿童，SN-MP>35?；
⑤上气道狭窄；
⑥受试者依从性良好，签署知情同意，能按要求配合矫治并定期复查。

Inclusion criteria

① 12-18 years old, no limitation of gender;
②Protrusion deformity, poor chin shape, mandibular retraction, ANB>5?;
③The maxillary bone development is basically normal;
④Children whose vertical growth type is high angle tendency, SN-MP>35?;
⑤The upper airway is narrow;
⑥The subjects have good compliance, signed the informed consent, and can cooperate with the correction as required.

排除标准：

①有颞下颌关节紊乱症状者；
②金属过敏患者；
③并发全身疾病患者。

Exclusion criteria：

①Patients with symptoms of temporomandibular joint disorder;
②Patients with metal allergy;
③Patients with systemic diseases.
