研究目的：

术后谵妄（postoperative delirium，POD）是由各种原因引起的术后 24~72 h 发生的急性脑器质性综合征，通常是急性起病、病程波动的精神状态，其特征是意识水平变化，定向力、注意力、思维、精神运动性行为以及情感方面等改变。研究表明口腔颌面外科术后谵妄发生率可达25%。POD的发生对患者预后会产生不良影响，如导致患者住院时间延长，增加患者术后病死率，以及增加术后早期认知功能障碍等。目前POD的病因尚不明确，可能是多种易感因素和诱发因素所引起的。近期国外指南和专家共识认为，POD 是由于手术创伤、麻醉应激等多种外在因素诱发和加剧患者中枢神经系统病变造成的结果，与围术期脑氧供需变化有关。据报道，右美托咪定具有抗炎作用。本实验探讨右美托咪定对行口腔颌面外科手术老年患者术后谵妄的作用。  

Objectives of Study：

Postoperative delirium (POD) is an acute organic brain syndrome occurring 24~72 h after surgery caused by various reasons. It is usually a mental state with acute onset and fluctuating course of disease, characterized by changes in consciousness level. Changes in orientation, attention, thinking, psychomotor sexual behavior and emotion. Studies have shown that delirium can occur up to 25% after oral and maxillofacial surgery. The occurrence of POD will have adverse effects on the prognosis of patients, such as prolonged hospital stay, increased postoperative mortality, and increased early postoperative cognitive dysfunction. At present, the etiology of POD is not clear, which may be caused by a variety of predisposing factors and predisposing factors. According to recent foreign guidelines and expert consensus, POD is the result of central nervous system lesions induced and aggravated by various external factors such as surgical trauma and anesthesia stress, and is related to changes in perioperative brain oxygen supply and demand. Dexmedetomidine has been reported to have anti-inflammatory effects. This study was to investigate the effect of dexmedetomidine on postoperative delirium in elderly patients undergoing oral and maxillofacial surgery.

纳入标准：

择期全麻下行口腔颌面外科手术的老年患者

Inclusion criteria

Elderly patients undergoing elective oral and maxillofacial surgery under general anesthesia

排除标准：

痴呆、急诊手术、心动过缓、病态窦房结综合征、心脏传导阻滞；肝或肾功能衰竭，术前ASA分级≥III级

Exclusion criteria：

Dementia, emergency surgery, bradycardia, sick sinus syndrome, heart block; Hepatic or renal failure, preoperative ASA grade ≥III
