探讨有创-无创双模闭环通气撤机技术应用于撤机困难患者中的有效性及安全性  

Objectives of Study：

to explore the effectiveness and safety of dual-mode closed loop weaning strategy in difficult weaning patients

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）至少经历48小时有创机械通气；（2）气管切开后SBT试验失败，预计短期内不能撤机；（3）年龄≥18岁，≤85岁；（4）意识清楚，格拉斯哥评分≥13分；（5）有自主呼吸能力；（6）PaO2/FiO2 >150；（7）无上气道阻塞；

Inclusion criteria

(1) Experienced invasive mechanical ventilation for at least 48 hours; (2) SBT failed after tracheotomy, mechanical ventilation is not expected to be removed in a short time; (3) ≥18 years old and ≤85 years old; (4) Clear consciousness, Glasgow coma score ≥13; (5) autonomous respiration ability; (6)PaO2/FiO2 >150; (7) without upper airway obstruction

排除标准：

（1）面部或颅骨外伤或手术；(2)面部异常；(3)近期胃或食管手术；(4)上消化道出血；（5）气胸患者；（6）缺乏配合；（7）无创机械通气禁忌症者；（8）终末期患者；（9）已纳入其他试验的患者；（10）拒绝参加本试验的患者

Exclusion criteria：

(1) facial or cranial trauma or surgery; (2) facial abnormalities; (3) Recent gastric or esophageal surgery; (4) upper gastrointestinal bleeding; (5) pneumothorax patients; (6) Lack of cooperation; (7) Contraindications to non-invasive mechanical ventilation; (8) terminally ill patients; (9) patients who have been included in other trials; (10) Patients who refused to participate in the study
