研究目的：

SCHWIND ATOS为全新的角膜屈光手术设备，它可以完成飞秒激光小切口基质透镜取出术（Smartsight），飞秒激光辅助准分子激光原位角膜磨镶术(FS-LASIK)的制瓣。它突破了传统飞秒激光设备不能定中心，缺乏眼球追踪系统，必须要多切削角膜基底等劣势，在尼泊尔对532只眼的临床应用已证实了其安全性和有效性，但其术后像差变化，术后干眼程度的变化规律尚不可知，故本前瞻性研究预探索Smartsight和FS-LASIK术后像差和干眼的变化规律。  

Objectives of Study：

SCHWIND ATOS is a new corneal refractive surgery device that can perform femtosecond laser small incision matrix lens removal (Smartsight) and femtosecond laser-assisted excision laser in situ keratoplasty (FS-LASIK). It breaks through the disadvantages of traditional femtosecond laser equipment, such as unable to determine the center, lack of eye tracking system, and need to cut more corneal base. Its safety and effectiveness have been proved by clinical application in 532 eyes in Nepal. However, the changes of postoperative aberration and postoperative dry eye degree are still unknown.Therefore, this prospective study explores the changes of aberrations and dry eyes after Smartsight and FS-LASIK.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18周岁及以上的近视、散光患者；
2.术前在充分理解的基础上，患者本人或必要时家属须共同签署知情同意书；
3.符合屈光度数：相对稳定（在过去1年内屈光度数变化≤0.50D）。等效球镜度数[-0.50D至-12.50D]；球镜度数[-0.50D至-10.00D]；柱镜度数[-5.00D至+5.00D]；
4.角膜：透明无明显云翳或斑翳；角膜地形图检查形态正常，无圆锥角膜倾向；
5.无其他眼部疾病和（或）影响手术恢复的全身器质性病变；
6.经术前检查无手术禁忌证者。

Inclusion criteria

1. Patients with myopia and astigmatism aged 18 years and above;
2. On the basis of full understanding before surgery, the patient or his family members must sign the informed consent form when necessary;
3. Compliant diopter: relatively stable (change in diopter within the past 1 year <= 0.50D). Equivalent spherical power [-0.50D to -12.50D]; spherical power [-0.50D to -10.00D]; cylinder power [-5.00D to +5.00D];
4. Cornea: transparent without obvious cloudiness or spotting; corneal topography examination is normal, no keratoconus tendency;
5. No other eye diseases and/or systemic organic lesions that affect the recovery of surgery;
6. Those who have no contraindications to surgery after preoperative examination.

排除标准：

1.患者在水平位置上无法平躺；
2.患者理解能力有问题，不能给予知情同意；
3.患者不能接受局部或表面麻醉；
4.怀孕或处于哺乳期；
5.不稳定时情绪低落或焦虑不安；
6.动机不当或期望不合理；
7.胶原血管、自身免疫或免疫缺陷疾病；
8.单纯疱疹病毒感染，例如带状疱疹；
9.糖尿病；
10.严重的局部感染或过敏性疾病，例如：角膜炎；睑缘炎；严重的过敏性眼病，需要定期治疗；
11.以前的疗法，例如放射状角膜切开术；
12.可能影响治疗区域的基质内切削精度的角膜疤痕；
13.治疗中使用影响伤口愈合的药物，例如：类固醇；抗代谢药；免疫抑制剂；
14.治疗中使用的药物对眼部有副作用，例如：异维A酸（Accutane）；盐酸胺碘酮（Cordarone）；
15.经研究者判定，受试者存在不可控的全身性疾病而无法参加实验。

Exclusion criteria：

1. The patient cannot lie flat in a horizontal position;
2. The patient has problems with understanding and cannot give informed consent;
3. The patient cannot receive local or topical anesthesia;
4. Pregnant or breastfeeding;
5. Depressed mood or anxiety when unstable;
6. Improper motives or unreasonable expectations;
7. Collagen vascular, autoimmune or immunodeficiency diseases;
8. Herpes simplex virus infection, such as herpes zoster;
9. Diabetes;
10. Severe local infection or allergic disease, such as: keratitis; blepharitis; severe allergic eye disease, requiring regular treatment;
11. Previous therapy, such as radial keratotomy;
12. Corneal scarring that may affect the accuracy of intrastromal ablation in the treatment area;
13. Drugs that affect wound healing are used in treatment, such as: steroids; antimetabolites; immunosuppressants;
14. Drugs used in the treatment have side effects on the eyes, such as: isotretinoin (Accutane); amiodarone hydrochloride (Cordarone);
15. According to the investigator's judgment, the subject has uncontrollable systemic disease and cannot participate in the experiment.
