研究目的：

1. 主要目的：本课题将结合16S rDNA扩增子测序、非靶向代谢组学手段解析呼吸道菌群在不同转移潜能非小细胞肺癌中的结构与功能特征，筛选与非小细胞肺癌演进相关的主要菌种及代谢物，并利用培养组学及质谱方法进行菌种纯化培养和鉴定。通过体外细胞培养及肿瘤血行转移小鼠模型，研究呼吸道菌群代谢对宿主呼吸道上皮细胞、非小细胞肺癌细胞、局部免疫细胞群的表型影响及机理，从而为非小细胞肺癌病理进化过程中的呼吸道菌群的作用提供新的理论依据，为肺癌的精准防治奠定基础。 2. 次要目的：鉴定不同CT影像表现的特征性呼吸道菌群及代谢物，探索潜在新型肺癌诊断标志物。  

Objectives of Study：

1.Primary objective: This study will explore the structural and functional characteristics of respiratory microbiota in non-small cell lung cancer of different metastatic potential by techniques such as 16S rDNA amplicon sequencing and untargeted metabolomics so as to filter main bacterial taxa and metabolites which are associated with the evolution of non-small cell lung cancer, and to culture and identify those purified bacterial strains by culturomics and mass spectrometry. Further, the current study will investigate the effects and mechanism of respiratory microbiota and their metabolites in different phenotypes of host respiratory epithelial cells, non-small cell lung cancer cells, local immune cells, so that provide new theories for the roles of respiratory microbiota in the pathogenesis of non-small cell lung cancer and lay the foundations for precise prevention and treatments for lung cancer. 2.Secondary objective: The study is also sought to identify distinctive respiratory bacterial taxa and metabolites in different computed tomography manifestations, thus exploring potential new diagnostic biomarkers for lung cancer.

纳入标准：

训练队列纳入临床特征（性别、年龄、吸烟史、基础疾病等）匹配的经病理活检确诊的原发性肺腺癌、肺鳞状细胞癌及肺良性疾病（结核瘤、支气管扩张、错构瘤、真菌感染等）患者，其中早期转移组（T分期≤T2a、N分期≥N1，低分化）、局部侵袭组（局部晚期无淋巴结及远处转移肺癌）（T分期≥T3、N分期=N0，高-中分化）及肺良性疾病组病例各30例。验证队列纳入符合上述纳入、排除标准患者45例（每组各约15例）。

Inclusion criteria

The training cohort of the study plan to recruit patients with matched clinical characteristics (gender, age, smoking history, underlying diseases, etc.) and pathological confirmed primary lung adenocarcinoma, primary lung squamous cell carcinoma and benign pulmonary diseases (tuberculoma, bronchiectasia, hamartoma, fungal infection, etc.). The included participants were divided into three groups: ①early metastasis group (T stage≤T2a, N stage≥N1, poor differentiation); ②local invasion group (T stage≥T3, N stage=N0, well to moderate differentiation); ③benign pulmonary disease group (30 cases for each group). The validation cohort will include 45 cases which conform to the above inclusion criteria (15 cases for each group).

排除标准：

①术前一月内抗菌药物使用史（包括口服或输注）；②术前化疗、免疫治疗、靶向治疗史；③需长期使用激素或免疫抑制剂疾病（如自身免疫病等）史；④需长期经呼吸道局部吸入药物疾病（如哮喘等）史；⑤含有多种病理类型的复合癌；⑥病灶＜1cm，或难以获得足够样本者；⑦存在胸膜转移或远处转移者。

Exclusion criteria：

①Oral or venous antibiotic consumption within one month prior to lung surgery; ②Preoperative chemotherapy, immunotherapy or molecular targeted therapy; ③Diseases which call for long-term use of hormones and immunosuppressors (e.g. autoimmune diseases); ④Diseases which call for long-term use of inhaling drugs (e.g. asthma) ; ⑤Lung cancer with mixed pathological types; ⑥Diameter of the lung lesion＜1cm or insufficient lesion sample; ⑦lung cancer with pleural metastasis or distal metastasis.
