研究目的：

主要目的：收集非重症哮喘、重症哮喘患者气道上皮细胞、平滑肌细胞及炎症细胞建立哮喘患者 Biobank， 分析重症哮喘、非重症哮喘患者及健康人群气道上皮细胞和气道平滑肌细胞在氧化应激、固有免疫 失调和表观遗传方面的差异，从而为哮喘表观分型及新型治疗药物提供基础。  

Objectives of Study：

Main purpose: To collect airway epithelial cells, smooth muscle cells and inflammatory cells from patients with non-severe asthma and severe asthma to establish a Biobank for asthma patients, and to analyze the relationship between airway epithelial cells and smooth muscle cells of airway epithelial cells and airway smooth muscle cells in patients with severe asthma, non-severe asthma and healthy people. Innate immune dysregulation and epigenetic differences, thus providing the basis for asthma epiphenotyping and novel therapeutic agents.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

重症哮喘组：
1.治疗情况：高剂量吸入糖皮质激素（ICS）或口服 糖皮质激素（OCS)（氟替卡松>1000mcg或同等 剂量），同时另一种哮喘控制用药；
2.哮喘控制情况：根据 GINA 指南哮喘未控制，在过去 4 周有以下 3 条以上的症状：
(1)白天症状≧2 次/周；
(2)活动受限；
(3)夜间症状≧1/周；
(4)缓解用药的次数≧2 次/周；
(5)使用支气管扩张剂前 FEV1< 80%预计值和或频繁哮喘急性加重（≧2 次/ 年需要高剂量口服激素、维持剂量加倍 3 天以上或需要住院）。
3.哮喘诊断：
吸入400mcg沙丁胺醇后FEV1≧12%或绝对值≧200ml或存在气道高反应性或2 天 PEF 昼夜变化平均值>8%或ICS、OCS、LABA 和 SABA
减量过程中 1 月内 FEV1 下降≧ 12%和 200ml+反复发作气喘史。

轻至中度哮喘组
1.治疗情况：低至中剂量 ICS（<500mcg 氟替卡松或同等剂量）；
2.哮喘控制情况：根据 GINA 指南哮喘控制，在过去 4 周内：
(1)白天症状<2 次/周；
(2)活动无受限；
(3)无夜间症状；
(4)缓解用药的次数<2 次/周；
(5)使用支气管扩张剂前 FEV1> 80%预计值。
3.哮喘部分控制：在过去 4 周有 1-2 条以下的症状：
(1)白天症状≧2 次/周；
(2)活动受限；
(3)夜间症状≧1/周；
(4)缓解用药的次数≧2 次/周；
(5)使用支气管扩张剂前 FEV1< 80%预计值。
3.哮喘诊断：
吸入400mcg沙丁胺醇后FEV1≧12%或绝对值≧200ml或存在气道高反应性或2 天 PEF 昼夜变化平均值>8%或ICS、OCS、LABA 和 SABA
减量过程中 1 月内 FEV1 下降≧ 12%和 200ml+反复发作气喘史。

非哮喘对照组
1.治疗情况：无；
2.哮喘控制情况：非哮喘病人FEV1≧80%预计值；
3.哮喘诊断：无哮喘病史，无其它系统重大疾病史。

Inclusion criteria

Severe asthma group:
1. Treatment situation: high-dose inhaled corticosteroids (ICS) or oral corticosteroids (OCS) (fluticasone > 1000mcg or equivalent), and another asthma control medication;
2. Asthma control: According to GINA guidelines, asthma is not controlled, and there are more than 3 of the following symptoms in the past 4 weeks:
(1) Daytime symptoms >= 2 times/week;
(2) Activities are restricted;
(3) Nocturnal symptoms>=1/week;
(4) The frequency of relief medication >= 2 times/week;
(5) FEV1 < 80% predicted value before bronchodilator use and or frequent asthma exacerbations (>= 2 times/year requiring high-dose oral corticosteroids, doubling the maintenance dose for more than 3 days or requiring hospitalization).
3. Asthma diagnosis:
FEV1>=12% or absolute value >=200ml after inhalation of 400mcg salbutamol or presence of airway hyperresponsiveness or 2-day average PEF diurnal variation >8% or ICS, OCS, LABA and SABA
FEV1 decreased by >= 12% and 200ml + history of recurrent asthma within 1 month during dose reduction.

Mild to moderate asthma group
1. Treatment situation: low to moderate dose ICS (<500mcg fluticasone or equivalent);
2. Asthma control: Asthma control according to GINA guidelines, within the past 4 weeks:
(1) Daytime symptoms <2 times/week;
(2) Unrestricted activities;
(3) No nocturnal symptoms;
(4) The frequency of relief medication is less than 2 times/week;
(5) FEV1> 80% predicted value before using bronchodilator.
3. Partial asthma control: 1-2 of the following symptoms in the past 4 weeks:
(1) Daytime symptoms >= 2 times/week;
(2) Activities are restricted;
(3) Nocturnal symptoms>=1/week;
(4) The frequency of relief medication >= 2 times/week;
(5) FEV1 < 80% predicted value before the use of bronchodilators.
3. Asthma diagnosis:
FEV1>=12% or absolute value >=200ml after inhalation of 400mcg salbutamol or presence of airway hyperresponsiveness or 2-day average PEF diurnal variation >8% or ICS, OCS, LABA and SABA
FEV1 decreased by >= 12% and 200ml + history of recurrent asthma within 1 month during dose reduction.

non-asthmatic control group
1. Treatment status: none;
2. Asthma control: FEV1>=80% predicted value in non-asthmatic patients;
3. Asthma diagnosis: no history of asthma, no history of other major diseases of the system.

排除标准：

1. 目前仍吸烟或既往吸烟>10 年 ；
2. 使用支气管扩张剂后 FEV1<1.0L ；
3．合并其它系统严重疾病 ；
4. 有电子支气管镜检查禁忌征 ；
5. 不能签署知情同意书者。

Exclusion criteria：

1. Current or previous smokers >10 years;
2. FEV1<1.0L after using bronchodilators;
3. Associated with other systemic serious diseases;
4. Contraindications for electronic bronchoscopy;
5. Unable to sign informed consent.
