研究目的：

牙髓再生术是一种手术方式，常规根管预备消毒后，利用自体血、血小板浓缩物或者自体干细胞募集趋化牙齿根尖孔外的干细胞进入根管，分化形成形成牙髓样组织。富血小板纤维蛋白衍生物的牙髓再生因为更富含纤维蛋白和生长因子、另外不需要根尖刺激出血，牙髓再生效果优于自体血牙髓再生效果；同时避免了干细胞体外培养扩增可能出现污染、干性丢失、基因突变等多种可能。浓缩生长因子（concentrated growth factor，CGF）是第三代血小板浓聚物，相比第二代富血小板纤维蛋白( PRF)，富含更多三维网状结构的纤维蛋白基质及多种内源性生长因子。胶原作为一种天然支架材料，具有很好的生物相容和生物降解性能、在体内可以降解，免疫原性较低，是常用的天然组织工程材料，也是口腔临床常用的材料。研究发现，相比合成的可降解支架材料，胶原更适合牙髓再生。  

Objectives of Study：

Pulp regeneration is a surgical method. After conventional root canal preparation and disinfection, autologous blood, platelet concentrates or autologous stem cells are used to recruit stem cells outside the apical foramen of teeth into the root canal, and differentiate into pulp-like tissue. The pulp regeneration of platelet-rich fibrin derivatives is more abundant in fibrin and growth factors, and does not require root apical stimulation and bleeding, and the effect of pulp regeneration is better than that of autologous blood pulp regeneration; at the same time, it avoids the possibility of in vitro culture and expansion of stem cells. There are many possibilities such as pollution, dry loss, and gene mutation. Concentrated growth factor (CGF) is a third-generation platelet concentrate. Compared with the second-generation platelet-rich fibrin (PRF), it is rich in more fibrin matrix with three-dimensional network structure and a variety of endogenous growth factor. As a natural scaffold material, collagen has good biocompatibility and biodegradability, can be degraded in vivo, and has low immunogenicity. It is a commonly used natural tissue engineering material and a commonly used material in oral clinics. The study found that collagen is more suitable for pulp regeneration than synthetic degradable scaffold materials.

纳入标准：

年龄在18至65岁之间，除第三磨牙的所有牙齿（牙根已发育完全），存在不可复性牙髓炎或根尖周炎症状，X线显示根尖周正常、根尖周膜腔增宽或根尖阴影。

Inclusion criteria

All teeth between the ages of 18 and 65 with the exception of the third grinding tooth (the root is fully developed) have symptoms of irreversible pulpitis or root tip circumliumitis, and X-rays show normal root circumcision, widening of the root tip circumcision cavity, or root tip shadow.

排除标准：

牙齿存在根折；牙齿无法直接树脂修复；牙齿大面积缺损导致以后难以检查牙髓活力；牙齿缺损至龈下2mm以上；牙周袋深度超过3mm；无对颌牙存在；无邻牙接触；存在系统性疾病；存在严重牙周疾病，牙根松动II度以上；内吸收；外吸收；根管严重钙化和髓腔闭锁。

Exclusion criteria：

Teeth with root folding teeth can not directly resin repair teeth large area defects lead to difficulties in checking the vitality of the pulp, tooth defects to more than 2mm under the gums, periodontal bag depth of more than 3mm, no jaw-to-jaw presence, no adjacent tooth contact, the presence of systemic diseases, the presence of severe periodontal disease, tooth root looseness II degrees or more, internal absorption, external absorption, severe calcification of the root tube and myelin latching.
