研究目的：

与全麻插管下VATS相较，从脑细胞电活动、脑组织氧合、脑血流供应和神经反射层面的监测并指导Tubeless VATS实现最佳麻醉深度，以期实现胸外科术中安全、质量的前提下，减少麻醉并发症，加快术后康复，为患者提供更好的体验。  

Objectives of Study：

Compared with VATS under general anesthesia intubation, Tubeless VATS can be monitored and guided to achieve the optimal depth of anesthesia from the aspects of brain cell electrical activity, brain tissue oxygenation of cerebral blood supply and nerve reflection, so as to reduce anesthesia complications, speed up postoperative recovery and provide better experience for patients under the premise of safety and quality during thoracic surgery.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)中青年组（18-59岁）：术前血压、血糖正常；既往无脑缺血、脑梗死病史，无颅脑手术及脑功能障碍疾病史，无精神、心理障碍疾病及服药史；既往无胃肠道疾病及手术史，免疫相关病史；近3个月无严重疼痛及持续服用止痛药物史；近1年无酗酒史；无营养不良，无失明及失聪；术前心、肺功能正常，无麻醉药物过敏史；因肺癌入院行楔形切除或肺叶切除准备；术前头颅MRI、脑电图及脑血流检查无禁忌证；排除因素：①体重指数（body mass index，BMI）≥28kg/m2的肥胖患者；②美国麻醉协会麻醉风险分级Ⅲ级以上；③Mallampati气道分级≥3级；④血流动力学不稳定、凝血功能障碍；⑤胃肠道疾病存在高反流风险，胃肠功能紊乱；⑥严重的呼吸系统疾病，如支气管扩张、支管管哮喘；⑦精神系统疾病且不能合作患者；⑧有睡眠呼吸暂停病史。沟通后同意参与该实验组。
(2)老年组（≥60岁）：术前血压、血糖正常，无明显脑梗塞症状；既往无精神、心理障碍疾病及服药史；既往无胃肠道疾病及手术史；术前心、肺功能无手术禁忌证；因肺癌入院行楔形切除或肺叶切除准备；术前头颅MRI、脑电图及脑血流检查无禁忌证；排除因素：①体重指数（body mass index，BMI）≥28kg/m2的肥胖患者；②美国麻醉协会麻醉风险分级Ⅲ级以上；③Mallampati气道分级≥3级；④血流动力学不稳定、凝血功能障碍；⑤胃肠道疾病存在高反流风险，胃肠功能紊乱；⑥严重的呼吸系统疾病，如支气管扩张、支管管哮喘；⑦精神系统疾病且不能合作患者；⑧有睡眠呼吸暂停病史。沟通后同意参与该实验组。

Inclusion criteria

(1) Young and middle-aged group (18-59 years old): preoperative blood pressure and blood glucose were normal;No history of cerebral ischemia or cerebral infarction, craniocerebral surgery or brain dysfunction, and no history of mental and psychological disorders or medication;No previous history of gastrointestinal diseases, surgery, immune-related history;No history of severe pain or continuous analgesic medication in the past 3 months;No history of alcoholism in recent 1 year;No malnutrition, no blindness and deafness;Preoperative cardiac and pulmonary functions were normal, and there was no history of anaesthetic drug allergy.Admission for lung cancer for wedge or lobectomy preparation;Preoperative head MRI EEG and cerebral blood flow examination showed no contraindications.Exclusion factors: obese patients with body mass index (BMI) of 28kg/m2;American Society of Anesthesiology risk rating grade above;Mallampati airway grade 3;Hemodynamic instability and coagulation dysfunction;Gastrointestinal diseases have a high risk of reflux, gastrointestinal dysfunction;Severe respiratory diseases, such as bronchiectasis and bronchiectasis;Patients with mental system diseases and unable to cooperate;(2) The elderly group (60 years old): preoperative blood pressure and blood glucose were normal without obvious symptoms of cerebral infarction;No history of mental and psychological disorders or medication;No history of gastrointestinal diseases or surgery;No preoperative contraindications of cardiac and pulmonary function;Admission for lung cancer for wedge or lobectomy preparation;Preoperative head MRI EEG and cerebral blood flow examination showed no contraindications.Exclusion factors: obese patients with body mass index (BMI) of 28kg/m2;American Society of Anesthesiology risk rating grade above;Mallampati airway grade 3;Hemodynamic instability and coagulation dysfunction;Gastrointestinal diseases have a high risk of reflux, gastrointestinal dysfunction;Severe respiratory diseases, such as bronchiectasis and bronchiectasis;Patients with mental system diseases and unable to cooperate;A history of sleep apnea was communicated and agreed to participate in the experimental group.

排除标准：

手术禁忌证患者排除入组。
