提出老年导尿管相关尿路感染的精准治疗方案。  

Objectives of Study：

To propose an accurate treatment plan for urinary tract infection associated with urinary catheter in the elderly.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、年龄≥60 岁
2、符合导尿管相关尿路感染者（含膀胱造瘘）
导尿管相关尿路感染诊断标准：主要是指患者留置导尿管后，或者拔除导尿
管 48 小时内发生的泌尿系统感染。
临床诊断：患者出现尿频、尿急、尿痛等尿路刺激症状，或者有下腹触痛、
肾区叩痛，伴有或不伴有发热。
病原学诊断：在临床诊断的基础上，符合以下条件之一：
①清洁中段尿或者导尿留取尿液（非留置导尿）培养革兰阳性球菌菌落数≥
104
cfu/ml，革兰阴性杆菌菌落数≥105
cfu/ml。
②耻骨联合上膀胱穿刺留取尿液培养的细菌菌落数≥103
cfu/ml。
③新鲜尿液标本经离心应用相差显微镜检查，在每 30 个视野中有半数视野见
到细菌。
④经手术、病理学或者影像学检查，有尿路感染证据的。

Inclusion criteria

1. Age ≥60 years old.
2, in line with catheter-related urinary tract infection (including cystostomy).
Diagnostic criteria of catheter-related urinary tract infection: mainly refers to the patients after indwelling catheter, or pulling out catheter.
Urinary system infection occurred within 48 hours.
Clinical diagnosis: the patient has urinary tract irritation symptoms such as frequent micturition, urgent micturition and dysuria, or has lower abdominal tenderness.
Pain in the kidney with or without fever.
Etiological diagnosis: on the basis of clinical diagnosis, one of the following conditions is met:
①clean the urine in the middle section or retain the urine through catheterization (without indwelling catheterization) and culture the colony number of gram-positive cocci ≥104cfu/ml, and the colony number of gram-negative bacilli ≥105cfu/ml.
② The bacterial colony number of urine collected by bladder puncture above pubic symphysis was ≥103cfu/ml.
③ Fresh urine samples were centrifuged and examined by phase contrast microscope, and bacteria were found in half of every 30 visual fields.
④ After operation, pathology or imaging examination, there is evidence of urinary tract infection.

1、相关资料不全或研究期间死亡者
2、拒绝签署知情同意书
