研究目的：

本研究旨在评估胆囊管受累能否作为预测因素以帮助III型和IV型肝门部胆管癌的手术决策。  

Objectives of Study：

The aim of this study was to evaluate whether cholecystic duct invasion could be used as a predictor to help clinical decision-making.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）首次大范围肝切除在我院进行，且术后病理检查证实为肝门部胆管癌；（2）肿瘤Bismuth-Corlette分型为III型或IV型；（3）全身情况良好，无严重心、肺、肾等重要器官的器质性病变；（4）术前肝功能Child-Pugh分级为A级，或经短期护肝治疗后肝功能分级恢复至A级的患者；（5）残肝体积大于标准肝体积的40%且ICG 15分钟滞留率小于10%。

Inclusion criteria

(1) the first extended hepatectomy was performed in our hospital, and postoperative pathological examination confirmed HCCA; (2) HCCA was type III or IV classified by Bismuth-Corlette classification; (3) patients was generally well without serious organic lesions of heart, lung, kidney and other important organs; (4) the Child–Pugh classification status was class A before surgery, or recovered to class A after short-term liver protection therapy; (5) residual liver volume was more than 40% of standard liver volume and indocyanine-green retention rate at 15 minutes (ICG15) was less than 10%.

排除标准：

（1）术前进行过放疗、化疗等其他抗肿瘤治疗；（2）合并其他恶性肿瘤；（3）术后未按照全新条目式病理报告模式评估是否达到R0切除；（4）术前胆囊缺如；（5）患者的临床相关数据缺失。

Exclusion criteria：

(1) had received other anti-tumor treatments such as radiotherapy or chemotherapy before operation; (2) complicated with other malignancies; (3) R0 resection was not evaluated according to the new itemized pathological report mode; (4) absence of gallbladder before operation; (5) clinical data related to patients were missing.
