研究目的：

本研究旨在通过社区家庭医生进行的干预提高2型糖尿病患者的自我管理能力和药物依从性，为加强社区糖尿病管理、提高糖尿病患者自我管理能力和药物依从性和完善糖尿病防治工作提供参考依据。  

Objectives of Study：

This study aims to improve the self-management ability and drug compliance of patients with type 2 diabetes through the intervention of community family doctors, and provide a reference for strategies to strengthen the management of diabetes in the community, improve the self-management ability and drug compliance of diabetic patients, and improve the prevention and treatment of diabetes.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄在55周岁到78周岁之间；已经确诊为 2 型糖尿病,病程半年及以上的患者；至少服用一种口服降糖药进行治疗；意识清楚、有生活自理能力和配合能力，病情稳定；具有一定的运动能力，可以进行治疗所需的基本运动在；具有听说读写等能力，能进行有效沟通;知情、自愿参加并签署知情同意书；可以使用手机和阅读短信。

Inclusion criteria

Aged between 55 and 78 years; patients who have been diagnosed with T2DM with a course of 6 months or more; take at least one oral hypoglycaemic drug for treatment; no cognitive problems; ability to self-manage their lives and willingness to cooperate; stable condition; ability to engage in exercise; ability to perform basic exercises required for treatment; ability to listen, speak, read, write, etc., and ability to communicate effectively; informed of the study purpose and willing to participate and sign an informed consent form; ability to use a mobile phone and read short message service (SMS) communications.

排除标准：

糖尿病合并有严重的并发症，如严重糖尿病肾病、糖尿病性视网膜严重病变等；伴其他严重的身体疾病；有精神异常和认知障碍的患者；特殊类型糖尿病、妊娠期糖尿病等患者；同时患有肿瘤并且半年内接受化疗、放疗等治疗的患者；正在参加其他研究课题的患者或拒绝参加研究的患者。

Exclusion criteria：

Severe diabetic nephropathy or severe diabetic retinopathy; presence of other serious physical diseases; mental disorders or cognitive impairment; special types of diabetes (e.g., gestational diabetes); tumours or chemotherapy, radiotherapy, and other treatments in the last 6 months; participation in other research studies; declined to participate in the research.

试验所选取的两个区的所有社区医院都将被编号。然后使用计算机生成的随机数，随机选择社区医院。使用西安市社区健康档案管理系统，导出所选社区医院老年人健康信息，从1到n排序。通过随机数软件生成204个号码的相应老年人作为样本。
