研究目的：

主要研究目的：1、研究艾司氯胺酮是否能改善患者围术期焦虑抑郁 次要研究目的：1、艾司氯胺酮对围术期细胞免疫的影响 2、分析艾司氯胺酮作用下患者围术期焦虑状态与细胞免疫二者的相关性，为改善患者围术期抑郁焦虑状态，保护细胞免疫功能，减少术后并发症，利于实施舒适化医疗提供临床参考  

Objectives of Study：

Main research objectives: 1. To study whether esketamine can improve perioperative anxiety and depression. Secondary research objectives: 1. Effect of esketamine on perioperative cellular immunity. 2. To analyze the correlation between perioperative anxiety and cellular immunity under the action of esketamine, so as to provide clinical reference for improving perioperative depression and anxiety, protecting cellular immune function, reducing postoperative complications and implementing comfort medicine

纳入标准：

纳入标准：
①年龄范围：20-60岁的成年人
②择期腹腔镜胆囊手术患者
③意识清晰，精神无异常，初中及以上文化水平，可独立准确完成量表的评估
④美国麻醉医师协会（ASA）分级I-II级
第I级：体格健康，发育营养良好，各器官功能正常；
第II级：除外科疾病外，有轻度并存病，功能代偿健全
⑤术前阿姆斯特丹术前焦虑和信息量表(APAIS）得分为11-20分的轻中度焦虑抑郁患者

Inclusion criteria

Inclusion criteria:

① Age range: 20-60

② Patients undergoing elective laparoscopic cholecystectomy

③ Clear consciousness, no mental abnormality, junior high school and above education level, can complete the evaluation of the scale independently and accurately

④ American Society of anesthesiologists (ASA) grade I-II

Grade I: physical health, good development and nutrition, and normal function of various organs;

Level II: except for surgical diseases, there are mild coexisting diseases and sound functional compensation

⑤ Patients with mild to moderate anxiety and depression with preoperative Amsterdam preoperative anxiety and information scale (APAIS) score of 11-20

排除标准：

排除标准：
①既往诊断为焦虑症、抑郁症、精神分裂症患者；
②APAIS表得分小于10或大于20分的患者
③心、脑、肾、肝等器官功能衰竭者；
④严重语言、听力障碍与智力发育迟滞者；；
⑤预计术中发生大出血（出血量≥30%全身血量），生命体征不平稳；
⑥既往有免疫系统疾病史；
⑦对艾司氯胺酮有过敏反应；
⑧术前高血压病未经药物控制，严重高血压：收缩压高于180mmhg
⑨拒绝纳入临床观察者

① Patients previously diagnosed with anxiety, depression and schizophrenia;

② Patients with APAIS score less than 10 or greater than 20

③ Heart, brain, kidney, liver and other organ failure;

④ Severe language and hearing impairment and mental retardation;;

⑤ It is expected that there will be massive bleeding (bleeding volume ≥ 30% of whole body blood volume) and unstable vital signs during the operation;

⑥ Previous history of immune system diseases;

⑦ Allergic reaction to esketamine;

⑧ Preoperative hypertension without drug control, severe hypertension: systolic blood pressure higher than 180 mmHg

⑨ Refusal to be included in clinical observers

由研究者用SPSS 23软件设置种子数123456，产生134个随机数，按生成的随机数大小由小到大排序，前67名进入对照组，后67名进入对照组

试验药物由不参加研究的麻醉护士发放，麻醉护士进行药品登记。麻醉医生及患者本人不知道随机分组的结果，负责麻醉的麻醉医生对患者进行术前及术后的回访并录入数据于数据库。
