研究目的：

本研究的主要目的是比较微骨折术和软骨下骨钻孔术这两种临床常规使用的手术对于距骨中小面积高级别骨软骨损伤患者治疗的有效性和安全性。  

Objectives of Study：

The primary objective of this study was to compare the efficacy and safety of microfracture and subchondral drilling, two procedures routinely used in clinical practice, in patients with small and high-grade osteochondral injuries in the talus.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.局灶性的踝关节不适症状，运动可加重；
2.经X线或（和）MRI证实有单一局部的OLT；
3.MRI上Hepple分型在IV-V型；
4.年龄在18-60岁；
5.软骨损伤直径 < 1.5 cm、面积 < 2.0 cm2且深度＜0.5cm；
6.保守治疗3个月无明显改善。

Inclusion criteria

1. Focal ankle discomfort symptoms, which can be aggravated by exercise;
2. Single local OLT confirmed by X-ray or (and) MRI;
3. Hepple classification on MRI is in IV-V type;
4. Aged 18-60 years;
5. Cartilage injury diameter < 1.5 cm, area < 2.0 cm2 and depth < 0.5 cm;
6. Conservative treatment for 3 months has no obvious improvement.

排除标准：

1.双侧踝关节损伤；
2.既往有踝关节骨折或其他外伤及手术史；
3.合并有踝关节肌肉萎缩或足踝部畸形及患侧肢体其他神经肌肉疾病。

Exclusion criteria：

1. Bilateral ankle injury;
2. History of ankle fracture or other trauma and surgery in the past;
3. Combined with ankle muscle atrophy or ankle deformity and other neuromuscular diseases of the affected limb.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据及时、完整、正确、清晰地载入病例报告表。经过监察员审核、签字后的调查表应及时送交临床研究数据管理员。录入采用相应的数据库系统双人双机录入，之后对数据库进行两遍比对，期间若发现问题及时通知监查员，要求研究者做出回答。他们之间的各种疑问及解答应当采用疑问表形式，疑问表应保存备查。病例报告表在按要求完成数据录入和核查后，按编号的顺序归档保存，并填有检索目录等，以备查考。电子数据文件包括数据库、检查程序、分析程序、分析结果、编码本和说明文件等，应分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。所有原始档案应按相应规定内的期限保存。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

According to the original observation records of the subjects, the data were timely, complete, correct, and precise in the case report form. The inspector reviewed and signed the questionnaire should be sent to the clinical Research Data Manager on time. The corresponding database system was used for input, and then the database was compared twice. If any problem was found, the supervisor was informed in time, and researchers were required to answer. Questions and answers between them shall be in the form of a question sheet, which shall be kept for future reference. The database will be locked at the same time. After completing data entry and verification as required, the case report form shall be archived, saved in a sequence of numbers, and filled with a retrieval catalog for future reference. Electronic data files, including databases, inspection procedures, analysis procedures, analysis results, codebooks, and explanatory files, should be kept separately. Multiple backup files should be kept on different disks or recording media to prevent damage. All original files shall be kept for the period specified accordingly.
